F. Rogge and R. F. Aiello of the NRC Region II office from June 9 te July 7,1989. 'In the enclosure, transcription of the violation precedes GPC's response.

ENCLOSURE V0GTLE ELECTRIC GENERATING PLANT NRC NOTICE 0F VIOLATION 50-424/89-19-01 ' AND GPC RESPONSE

" Technical Specification 6.7.1.a requires that written procedures be implemented covering activities delineated in Appendix "A" of Regulatory Guide 1.33, Revision 2, . February 1978.

Operations Procedure 10001-C, Section 3.3, specifies Table 1 for inspection criteria when performing rounds Paragraph 5.0 of 10001-C further states that the operator on duty is reponsible for all charts in his area which includes assuring that the chart is operating properly.

Contrary to the above, ~ on June 14, 1989, the NRC identified that improper control room rounds were being conducted. The findings include failure to verify proper operation of the refueling water storage tank level channel II and containment pressure chanr:e1 IV.

This is a Severity Level IV violation (Supplement 1)."

RESPONSE TO VIOLATION 50-424/89-19-01 Admission or Denial of Alleged Violation:

The violation occurred as stated.

Reason For The Violation The cause of the violation was personnel error and procedural violation.

Operations Administrative Procedure 10001-C, "Logkeeping", Rev. 8 states that, "The operator on duty is responsible for all charts in his area. This includes assuring that each recorder chart is marked daily with time and date. Also, each chart should be checked for proper operation." On June 13, 1989 RWST Level Channel II (green pen) of chart recorder 1LR-990 stopped inking and was later stamped by a control room operator signifying that the recorder was operating properly. The next shift found and corrected the inking problem.

On June 10, 1989, Containment CNMT Pressure Channel IV (green pen) of chart recorder 1-PR-934 stopped inking and was stamped by eleven subsequent shift crews signifying that the recorder was operating properly. Upon discovery of the inking problem by the NRC inspector, the problem was corrected.

Both chart recorders are two pen recorders with red and green inkers. In both l cases, the green inkers located behind the red inkers ceased to ink.

I Complicating the visual inspection process, both recorder channels reflect the same value, i.e. the red and green traces lie on top of one another. From discussion with operators involved, it is apparent that the red trace was inadvertently mistaken as a composite of red and green inkers.

[ Corrective' Steps Which Have Been Taken and The Results Achieved:

An interoffice correspondence letter from Operations Management was. issued to '

all operators and supervisors .to make them aware of this violation. The 4 letter explained the problem associated with two pen recorders that have channels which reflect the same value. Additional care was urged when checking these recorders. <

Corrective Steps Which Will' Be Taken To Avoid Further Violations:

1) Increased Operations Management attention will be provided by routine inspections of control room chart recorders.

2). A copy of this NRC violation and response will be placed in the Operations ,

Reading Book to ensure all licensed individuals are aware of the  !
