Subject:. Haddam Neck Team Inspection Report No. 50-213/87-22'

This refers to the special operational' safety inspection conducted by a team

of Region I personnel led by Kenneth Murphy of this office on August 24--

September 4, 1987 at the Haddam Neck Plant, Haddam Neck, Connecticut of activities

authorized by NRC License No. OPR-61 and to the discussions of our findings held -

by Mr. Murphy with members of your staff at the conclusion of the inspection.

Areas examined during this inspection included systems and operator actions

selected from the accident sequences described in the "Probabilistic Safety

Study" (PSS) prepared by NUSCO. The inspection focused on the availability of

critical components as evidenced by plant conditions, operating experience, and

the implementation of surveillance and maintenance programs. In addition, the

inspection examined the ability of the Haddam Neck staff to use procedures and

equipment to respond to and recover from certain' postulated accident sequences.

These areas are described in the NRC Region I Inspection Report which is enclosed

with this letter.

Within these areas, the inspector examined selected procedures and representative

records, conducted interviews with personnel, inspected plant equipment, and

observed simulated emergency operations by plant personnel.

Overall, the results of the inspection indicate'that the plant staff and emergency

core cooling systems are capable of reliable response to the two domir, ant accident

sequences identified in the current PSS. This conclusion assumes that the main-

tenance initiatives and recent modifications to the charging pumps will reduce

down time and eliminate shaft breakage The inspection report summarizes other

positive initiatives by your staff including the recent involvement of your PSS

staff in the safety review of design projects at the formative stage.

Based on the results of this inspection, it appears that certain of your activities

were not conducted in full compliance with NRC requirements, as set forth in the

Notice of Violation, enclosed herewith as Appendix A. This violation has been

categorized by severity level in accordance with the General Statement of Policy

and Procedures for NRC Enforcement Actions, 10 CFR Part 2, Appendix C (Enforcement

Policy 1987). You are required to respond to this letter and in preparing your

response, you should follow the instructions in Appendix A. Additionally, you

are required to provide the details of a root cause analysis performed for this

violation and the actions derived from this analysis to preclude recurrence of

similar violations.

In addition, the team identified weaknesses in certain areas that warrant atten-

tion. These weaknesses were discussed with plant management and cominitments

were made for their correction. These commitments and the proposed completion

dates are summarized in the enclosed inspection report. In your response to

this report, please indicate any changes or discrepancies in the commitments or-

completion dates.

The responses directed by this letter and the accompanying Notice are not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, PL 96-511.
