NOTICE OF VIOLATION Connecticut Yankee Atomic Power Company Docket No. 50-213 Hartford, Connecticut 06141 License No. DPR-61 As a result of the inspection conducted on August 24 - September 4, 1987, and in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (E,forcement Policy) (1987),

the following violation was identified:

Section 6.8.1 of the Technical Specifications incorporated in License DPR-61, pertaining to the Haddam Neck Plant, requires the licensee to meet or exceed the requirements in Sections 5.1 and 5.3 of ANSI N18.7-1976. Section 5.3.2(6) of this Standard requires procedures to contain " limitations on the parameters being controlled" and where appropriate, " quantitative control guides should be provided".

Contrary to the above, the special maintenance procedure SPL 10.7-273 to perform a flow test of the Residual Heat Removal (RHR) System neither contained adequate limitations on controlled parameters nor provided appropriate quantitative control guides as evidenced by the overheating of RHR Pump "B" on December 21, 1986 caused by the throttling of the Component Cooling Water System that resulted in insufficient cooling water to the pump's Seal Water Cooler.

This is a Severity Level IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Connecticut Yankee Atomic Power Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, considera-tion will be given to extending this response time.
