Gentlemen:

Subject: Resident Inspection 50-213/86-01 (1/9/86 - 2/10/86)

.This transmits the findings of the routine NRC inspection of refueling and main-

tenance outage activities at the Haddam Neck Plant. These findings have been dis-

cussed with Mr. R. Graves of your staff.

The inspection identified Violations related to the administrative control of on .

going plant modifications. Please reply to these matters in accordance with the

enclosed Appendix A.

We are concerned that the root cause of the enclosed violations may be inadequate

training 'of engineering and construction personnel in the control of requirements

for implementation of plant modifications. Such a weakness was cited by you as

a causal factor for design change control problems exper_ienced at Haddam Neck dur-

ing the period 1979-1984. Consequently, in your response to Appendix A, please

address the effectiveness of your corrective actions for previous design control

problems in relation to the present training and qualification of personnel.

Detail 5.2 of the enclosed inspection report notes that four tubes in a steam

generator were not eddy current tested. We understand that you plan to justify

and request a one-time relief from Technical Specification requirements for eddy

current testing of these tubes and that your submittal concerning such relief has

been on hold within your organization. Because this relief is needed to resume

operations, we recommend that any such submittal be made as soon as practicable.
