APPENDIX A NOTICE OF VIOLATION Connecticut Yankee Atomic Power Company Docket No. 50-213 Hartford, Connecticut 06101 License No. DPR-61 As a result of the inspection conducted on January 9 - February 10, 1986, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the-following violations were identified:

A. Technical Specification 6.8; 10'CFR 50, Appendix B, Criterion V; and the Northeast Utilities Quality Assurance Program require activities affecting quality to be prescribed and accomplished in accordance with written proce-dures. Administrative Control Procedure ENG 1.7-17, Design Change Notices (DCNs) for Design Documents, requires that: (1) field changes be numbered using the Project Assignment (PA) or Plant Design Change Request (PDCR) number followed by a sequential . number, and (2). field changes be evaluated and checked to be significant changes or not. A PDCR revision is requic ed for ,

significant changes.

Contrary to the above:

1. Two DCNs for PDCR-787, Equipment Hatch Penetrations, and eight DCNs for 4
PDCR-789, Cavity number followed by Drain Piping,l number.were a sequentia not numbered Consequently, using the PA or PD no sequential listing of changes to these projects existed.
2. Field changes to.PDCR 789 were not adequately evaluated and checked for significance, in that DCNs C007-86, C029-86 and C032-86 were not checked as to their determination of significance; and DCNs C024-86, M008-86, M005-86 and M252-85, though checked as significant, had no PDCR revision initiated.

This is a Severity Level V Violation (Supplement I).

B. Technical Specification 6.8; 10 CFR 50, Appendix B, Criterion V; and the Northeast Utilities Quality Assurance Program require activities affecting quality to be prescribed and accomplished in accordance with written proce-dures. Administrative Control Procedures ACP 1.2-3.1, Preparation, Review and Disposition of Plant Design Change Requests (PDCRs) and ENG 1.7-11, Pre-operational Testing of Plant Modifications, require a test plan to be sub-mitted with each PDCR. The test plan shall be reviewed and approved to verify.

that the modification.will be ad,quately tested.

Contrary to the above, the test plan reviewed and approved with PDCR 785, Emergency Diesel Generator (EDG) Air Filters, would not adequately test this modification; in that the specified preoperational test would not ensure that all functional aspects of the EDG air start system were tested.

This is a Severity Level IV Violation (Supplement I).

. Pursuant to 10 CFR 2.201, Connecticut Yankee' Atomic Power Company is hereby re-quired to submit to this office, within thirty days of the letter which transmitted this Notice, a written reply including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is.shown, consideration will be given to extending this response time.
