Mississippi Power and Light Company        2                  Docket No. 50-416 Grand Gulf                                                    License No. NPF-29 Paragraph 6.6.2 of Administrative Procedure ( AP) 01-S-07-4 requires the 1 responsible engineer to update the as-built drawing list and ensure they are diitributed prior to returning the affected system to operation.
Paragraph 6.1.2 of AP 01-S-07-1 requires all maintenance work be accom-plished using written' and approved work instructions and be authorized by approved maintenance work order or maintenance planning and scheduling system task card.
Contrary to the above:
: 1. As of May 21, 1986, drawing M-1106A had not been updated to reflect design change package 84/4063 which was signed off as completed on February 21, 1986.
: 2. On April 7, 1986, no documented instructions were utilized when attempting to reset trip units on the low-low set logic for safety /
relief valves resulting in the inadvertent lif ting of safety / relief valve 1B21-F0518.
This is a Severity Level IV violation (Supplement I).
C. 10 CFR 50, Appendix B, Criterion II and the accepted QA program, MPL-TOP-1A, requires indoctrination and training programs for personnel performing quality-affecting activities which assure that personnel performing quality-affecting activities are trained and qualified in the principles and techniques of the activity being performed.
Contrary to the above, training of fuse indoctrination and identification was inadequate in that on April 30, 1986, while performing a tagout, an operator removed the wrong fuse resulting in the inadvertent startup of Division I combustible gas control system.
This is a Severity Level 11l violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Mississippi Power and Light Company is hereby required to submit to this Of fice within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including: (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
