Cement shall be placed in cement silos as designated by Plant Supervisor. Prior to cement unloading, air slides shall be checked for i B cement clods, and air slide adapters shall be firmly attached to rail car. 6.1.5 Cement received by truck shall not be unloaded until QC w Inspector has recorded cement temperature, truck seal 2 number, carrier identity and truck number. AGE:4 CT 11 l o.2.i Prior to daily production and periodically during daily operations, front end loader (FEL) operator shall inspect i loader for any oil or hydraulic fluid leaks and mud that might be on tires, loader bucket, or loader frame. Any oil or hydraulic fluid leaks shall be reported to Plant Supervisor for repair. If loader has oil or hydraulic fluid leaks. loader shall not be placed on any material pile or be used in a manner to contaminate tihe piles and if oil or hydraulic fluid falls on areas adjacent to stockpiles, no w material vill be unloaded on these areas until the oil or @ hydraulic fluid is cleaned up. 1.oader bucket, tires, and Z frame must be maintained in clean condition at all times.

A sketch or a memo / letter shall be maintained in cia the control room showing what material is stored in each g ground bunker.

Under normal C conditions the plant central mixer will be used as a shrink p; mixer; but if the aggregate gradation, changes or if any of E the materials overweighed beyond the scale tolerances, the 5 overweighed material shall be removed.or an additional s amount of material up to one cubic yard can be weighed, correcting for overweights, and added to mix. 4 6.4 Concrete Truck Drivers 6.4.1 Ever truck driver shall be instructed by Plant Supervisor or his designee as to pour location, truck routing, and delivery ticket content. Copy of ticket attached for reference. Overall Ticket Information: Ticket consists of a 5-parc serialized proprinted delivery ticket that includes the truck delivery ticket information and required digitally recorded information. The initial disposition of each ticket is printed on each copy and is as follows: Copy #1 - Plant Copy - To be retained at batch plant af ter batch is complete. Copy #2 - Driver's Delivery Copy - To be returned to batch plant after concrete delivery is completed. Each line of data consists of 25 characters or spaces for customer ID pour identification, six design data, etc. FIRST LINE: Customer name and pour location. Customer Name - ESASCO SES3 (11 spaces includes 1 blank space) Pour Location - A number to be supplied by Ebasco at time of initial l order placement. This number will be a 2-part number, the first part being up to 6 digits referring to the d'rawing number where the pour is shown, the second part of up to 6Lcharacters (numbers / letters) that

Sand Bin - This is the sand bin number that the sand was drawn from. The north sand bin is Sin Number 3 while the south sand bin is Bin Number 4 in the main plant while the backup plant sand bin-is Bin Number 2. Admix % Adjust - 1st - Basic mix designs as entered into the computer 2nd assumes a given dosage rate for the air entraining 3rd admixtare and for the Type A or D admixture. For each job using a given mix, the amount of admixture is controlled by this % Adjust selection section. As an example, if a given six has 2 ox. of air per cubic yard and 3 ou per sack of cement, Type D admix is the given dosage rate and

On the primary plant, the north silo is Number i south silo is Number 2. For the backup plant, the silo is always Number 1. Water - Net weight recorded in pounds for water is recorded in water block along with the type of water being used. Water type Number 1 is used when regular tap water is being used and N.aber 2 is used when processed water (heated or chilled) is being ased. Water batched is based on mix design adjusted by the slump adjust indicator less the amount of free moisture in the concrete sand as indicated by the moisture meter and less any ice added to mix.

This information is a supplement to the ready mix delivery ticket information sheet that was sent to Ebasco on June 13, 1975 copy attached for refarence. On Line 2 there are three blocks for aggregate, cement, and water " sero" scale readings. The actual scale readings will be printed in these blocks. There is a " sero" interlock that prevents any batch' cycle from starting if the scale j reading on any batcher is our by more than three scale divisions. Otherwise, the automation will print the actual scala reading and will indicate either plus or minus for the value printed. On Line 3 the not weights for each material is printed in the appropriate block; this weight will correct for the sero scale readings. l' As an example. let's assume the following mix designi Cement - 4978 Agg 1 - 11098 l i Agg 2 - 733W Agg 3 - 1277f (SSD) j Water - 258f Air - 1 os.
