WATERFORD SES UNIT NO. 3 CONTRACT NO. W3-NY-4 1.0' PURPOSE The purpose of this procedure is to establish guidelines for the repair, in-spection, documentation and verification of the Base Mat Foundation beneath Columns H10A and Hila which are on Placement 499503-19.

2.0 SCOPE This procedure shall govern the repair, inspection, documentation and veri-fication of subject column foundations for further construction.

4.0 DEFINITIONS 4.I Engineer:

Ebasco Services, incorporated.

5.0 RESPONSIBILITY 5.1 Ebasco Services, Inc shall be responsible for documentation review and determining number, size and location of cores for repair verification.

5.2 J. A. Jones Construction Company shall be responsible for the coring, drilling, purchasing, receiving and acceptance of grouting material, grouting operation, Quality Verification inspection, documentation and preparation of documentation for review by the Engineer.

5.3 J. A. Jones Construction Company shall have a quallfled engineer present during all grouting operations who shall be in responsible charge of the work.

MATERIAL QUAllFICATION 0

It is recognized that all of the requirements as set forth in the J. A. Jones quality Assurance Manual, Procedures and Instruction, Procedure POP-N-710, PRELIMINARY EVALUATION OF BlD CANDIDATES,tand POP-N-711, PRE-CONTRACT AWARD EVALUATION, may not be met by,the vendor.

If the qualification criteria referenced above are not met, the following procedure shall be followed as an alternate for the qualification of epoxy material required for the repair of Placement 499503-19 in accordance with the Engineer's requirements as stated in Paragraph 6.7 below.

1 6.1 J. A. Jones' Quality' Assurance shall conduct a vendor plant survey audit per POP-N-710 and POP-N-711 as ipplicable to the services provided by the vendor.

6.2 During the vendor plant survey audit particular attention shall be shown to the testing and verification facilities in regards to appearance, equipment, equipment calibration and documentation of tes.t results thus. developing a level of confidence in the plant facility.

R-1 6.3 During vendor plant survey audit, collect at random a representative quantity fo certified test results of numbered lots.

From these certifications, make a study of the variance between the certifications 4

to arrive at a level of confidence in the vendor testing facility.

R-1 6.4 From the sampled separat.e certified test reports and plant survey study, t

arrive at a level of confidence as to whether the lot or material purchased for the repair will meet the manufacturer's requirements.

If the 1evel of confidence is high, as demonstrated by the collected

. data proving a high level of confidence that the vendor has and can consistently produce acceptable material then proceed with the repair without prior testing of the lot or lots of the meterial to be used in the repa i r.

Test the lot of material being used per Paragraph 6.7 below 6.5 Should the level of confidence of material acceptance be low then proceed to perform the tests as required by 6.7 below and receive the test results from an independent testing laboratory prior to beginning the repair.

GROUTING 8.1 A minimum of four injection machines shall be on hand for the grouting operat ion.

8.2 Start the grouting operation at the northern corners of Column Hil A witfi-hole Nos. Hila-1 and Hila-4.

Observe all !! oles for influence of grout.

.lf grout is observed entering hole No. Hil A-7, prepare hole for grouting and start grouting with third machine, if grout is observed flowing into other holes, add aggregate to hole as influence of grout requires.

Continue pumping these three holes until refusal at 180 PSI.

Continuously observe a'll holes and add aggregate as required.

When aggregate level reaches a point requiring installation of packer, then install packer, leaving vent open until all air escapes, then close vent.

When any hole which is being pumped reaches refusal, then proceed to the next hole designated and prepare for grouting.

Begin grouting when directed by the J. A. Jones Engineer.

Continue grouting from north to south in accordance with above procedure until all holes' in Column.

Hil A are grouted.

j 8.3 Start the grouting operation at the northern corners of Column'H10A' with hole Nos. H10A-3 and Hl0A-7.

Observe all holes for influence of grout.

If grout is observed entering hole No. H10A-9, prepare hole for grouting and start grouting with third machine.

If grout is observed flowing into other holes add aggregate to hole as influence of grout requires.

Continue pumping these three holes until refusal at 180 PSI.

Continuously observe all holes and add aggregate as required.

When aggregate level reaches a point requiring Installation of packer, then install packer, leaving vent open until all air escapes then close vent.

When any hole which is being pumped reaches refusal, then proceed to the next hole designated and prepare for grouting.

Begin grouting when directed by the J. A. Jones Engineer.

Continue grouting from north to south in accordance with above procedure until all holes in Column H10A are grouted.

8.4 Sump liners lying on Placement 19 shall be drilled and tapped in accordance with Appendix C.

If grout is observed in any of these holes or the existing hole in the bottom of the sump, the grouting operation shall be stopped and the subject observation hole shall be plugged with a screw plug.

Move to another grout hole allowing a miniinum of ten (10) minutes for grout to seal passage between grout i

hole and sump liner.

Continue this procedure during the entire grouting operation..

Drill and tap holes in sump liner as shown above.

As grout is injected into the slab, observe these holes.

If grout is observed entering any of these holes or existing hole in bottom of sump, stop grouting operation imediately.

Plug hole which grout was observed flowing into, with screw plug.

Move grouting operation to another hole providing enough time for grout to set and close the passage from grout hole to sump liner.

Continue this procedure during all the column grouting operation.
