SEQUOYAH NUCLEAR PLANT SPECIAL REPORT 83-03 UNIT 1 Event Description Two mechanical penetrations in the auxiliary building (elevations 714 and 728) were breached for a period greater than seven (7) days. The penetrations were breached at 2100 EDST on 06/16/83 and both were resealed at 2240 EDST on 06/23/83.

Probable Cause The cacan of the event has been attributed to the lack of communication between personnel. The craft foreman who was in charge of the work requiring the penetrations to be breached took leave but failed to inform his relief of the breached penetrations. A routine inspection of work locations revealed the breached penetrations, and they were sealed at 2240 EDST on 06/23/83.

Corrective Actions Foremen have been instructed to post copies of all breaching permits in the general foremen's office so that all foremen will be aware of the status of all breached penetrations. A firewatch as required by technical specification 3.7.12 was initiated upon discovery.
