I.

FIRE BARRIER DOOR A-156A Dgantiption of CondiliDD On October 26, 1993, with Unit 1 in Mode 5 coming out of a refueling outage (Unit 1 Cycle 6) and Unit 2 in Mode 3 coming out of a maintenance outage, Fire Barrier Door A-156A was breached.

Door A-156A is a roll-down fire barrier accessing the Unit I reactor building access room from the refueling floor on Elevation 734.0 in.

The door was breached to permit temporary electrical cables to be routed from an existing card reader near the Unit 1 steam generator blewdown room (A-10) to the Unit i reactor access room for temporary installation of a personnel card-reader device. The card readers are used for personnel accountability of workers entering Unit 1 containment during the outage.

The door will be breached in excess of the technical specification (TS) allowable timeframe and is expected to be returned to a f unctional status in December 1993.

CarrentiYr_ Action _

In accordance with Limiting Condition for Operation (LCO) 3.7.12 Action Statement (a), the operability of the fire detectors on one side of the breach was verified, and an hourly fire watch was-established and will be maintained until the barrier is restored to a functional status.

This condition is being reported in accordance with TS Action Statement 3.7.12.a.

II.

FIRE BARRIER DOOR A-193 Description _of_Condillon On October 25, 1993, with Unit 2 in Mode 3 coming out.of a maintenance outage, Fire Barrier Door A-193 was breached. Fire Door A-193 connects the 2A and 2B 480-volt (V) board rooms on Elevation 749.0 of the. auxiliary building.- The door was breached open to allow the 2A 480-V board room air-conditioning unit to cool the 2B 480-V board' room because the 2-B board room' air-conditioning.

room chiller was out of service for maintenance. The door was-breached.in excess of the TS allowable timeframe and was returned to.
