.. ... ;j ENCLOSURE: l SEQUOYAH NUCLEAR P1 ANT UNITS 1 AND 2 SPECIAL REPORT 93-09 In support of the activities associated with two modifications to the L nuxiliary building heating and ventilation system, the following 19 active and passive fire protection impairments were. intentionally .,

initiated and carried out. Appropriate compensatory measures were  !

established and maintained in each case. The impairments were maintained E for a period of time that exceeded the allowable technical s'pecification (TS) timeframe. ,

i The two modifications will provide an A-train ventilation source to the i A-train vital inverters and battery chargers located in the Unit 1, 480-volt (V) board room (1B) and Unit 2, 480-V board room (2B). "

These conditions are being reported in accordance with TS Action .!

Statements 3.7.12.a 3.7.11.2.a, and 3.3.3.8.b.

I. EIRE _It00RSJ-1fLO.i_.A-194. AND A-1M i

Door A-180 is the adjoining door between the Unit 1, 480-V Board  ;

Rooms 1-A and 2-A in the auxiliary building on Elevation 749.0. ,

This door was breached for ventilation and traffic movement.

Door A-194 is the door accessing the Unit 2, 125-V Vital Battery-Room IV in the auxiliary building on Elevation 749.0. This door l was breached for_ ventilation and traffic movement. ]

Door A-195 is the door accessing the Unit 2, 1250-V Vital Battery ,

Room-III in the auxiliary building on Elevation 749.0. This door-was breached for ventilation and traffic movement.  ;

II. SERINKLER_SXSIIMS Unit _1 _125-V Vital Bat _t. cry Rooms I. II. III. and IV These rooms are in the auxiliary building on Elevation 749.0. The sprinkler systems for these rooms were made nonfunctional (isolated) to permit rerouting of the fire protection lines and relocation of the associated sprinkler heads to effectively ,

accommodate the additional heating, ventilating, and i air-conditioning (HVAC) ductwork being installed in the rooms. -

III. IIRE_ DETECTION Unit _2._125-V Vital. Bat.tery_Eneas III and IV The Unit 2, 125-V Vital Battery Rooms III and IV are in the {

auxiliary building on Elevation 749.0. The smoke detectors for these rooms (Fire Zones 253, 254, 255, and 256) were made nonfunctional to prevent false alarms that may be initiated because- l of the welding and other fumes being generated during the -i modification activities. i i

IV. EIRE _EARRIER_ HALLS '

A. A. hole was cut in an auxiliary building reinforced masonry wall serving as a fire barrier. This wall is located in the S column on Elevation 749.0 between Columns A-10 and A-11. A HVAC duct and fire damper will be installed in the wall.

B. A hole was cut in an auxiliary building reinforced masonry wall  !

serving as a fire barrier. This wall is located in the R column on Elevation 749.0 between Columns A-10 and A-11. A HVAC duct and fire damper will be installed in the wall. .i C. A hole was cut in an auxiliary building reinforced masonry wall' {

serving as a fire barrier. This wall is located in the S column on Elevation 749.0 between Columns A-8 and A-10. A .i HVAC duct and fire damper will be installed in the wall.

D. A hole was cut in an auxiliary building reinforced masonry wall serving as a fire barrier. This wall is located in the j A-12 column on Elevation 749.0 between Q and R column lines. A HVAC duct and fire damper will be installed in the wall.

E. A hole was cut in an auxiliary building reinforced masonry wall serving as a fire barrier. This wall is located in the j

A-11 column on Elevation 749.0 between Q and R column lines. A HVAC duct and fire damper will be installed in the wall. i F. A hole was cut in an auxiliary building reinforced masonry wall serving as a fire barrier. This wall is located in the S column on Elevation 749.0 between A-6 and A-8 column lines. )

A HVAC duct and fire damper will be installed.in.the wall.

G. A hole was cut in an auxiliary building reinforced masonry wall serving as a fire barrier. This vall is located in the i S column on Elevation 749.0 between A-3 and A-4 column lines.

A HVAC duct and fire damper will be installed in the wall. l 1

H. A hole was cut in an auxiliary building reinforced masonry wall  !

separating the Vital Board Rooms III and IV on  :

Elevation 749.0. This wall is located in Column A-ll between  !

Columns Q and R. The hole was necessary to reroute a fire  ;

These isolation valves isolate the vital battery rooms' sprinkler system and are normally. closed to prevent the inadvertent spraydown  ;

of the vital batteries. These valves were verified closed and

" hold order tags" are attached to the valves to prevent operation of the valves. The valves were taken out-of-service to isolate the fire-suppression lines to permit the existing lines and -sprinkler heads in the. rooms to be cut out and relocated. Rerouting the lines and relocating the sprinkler heads are necessary because the installation of the HVAC ductwork associated with the modifications '

interferes with the effectiveness of the existing fire suppression system.
