CERTIFICATE OF INSERVICE INSPECTION I, the undersigned, holding a valid commission issued by the National Board of Boiler and Pressure Vessel Inspectors and employed by the Hartford Steam Boiler Inspection and Insurance Company of Connecticut have inspected the items described in this Owner's Activity Report, and state that, to the best of my knowledge and belief, the Owner has performed all activities represented by this report in accordance with the requirements of Section XI.

The conditions observed, while rejectable, did not appear as fresh active boric acid and exhibited no apparent significant change from the previous observation.

Evaluation The quarter-inch thick plate had no signs of pitting or wastage, the liner has maintained complete integrity as per the original design. These areas are a source of borated water in the reactor pit area from the upper past cavity seal ring and floor when the cavity is flooded.

There are currently no indications /conditions that would appear to challenge the containment structural integrity of the structural integrity/leak tightness of the containment liner boundary during Modes 1-4 Conclusion WO 40801001-01 has been created to clean and coat deficient locations as required. This shall be implemented during PTN3-34 Refueling Outage.

Attachment 1 Engineering Evaluation: PTN-ENG-SESJ-23-004 Rev. 0 EC-298869 Rev. 0 Page 3 of 5 FORM OAR-1 OWNER'S ACTIVITY REPORT TABLE 1 ITEMS WITH FLAWS OR RELEVANT CONDITIONS THAT REQUIRED EVALUATION FOR CONTINUED SERVICE Examination Category and Item Description Evaluation Description Item Number F-A CCW Head Tank AR02455047 F1.30 Description Identified The following recordable indications and discrepancies were observed while performing an ASME Section XI VT-3 on the CCW Head Tank 3T216. 1) Corrosion was served on the bottom support lower nuts, 2) Multiple gaps were observed between the support plate and embedded baseplate where caulking has degraded, and 3) discrepancy was identified on the as-built drawing. An approximately 3/8" gap was observed on the lower support (North end),

where a weld was required by drawing.

Evaluation The corrosion identified on the lower nuts appears to be surface corrosion. It will not affect the ability of the bolt to transfer tension or reduce the shear capacity of the bolt. The bolts are fully engaged with adequate thread engagement. The bolts described are in place to fasten a 1"X 11" Bent Plate. Per Drawing 5613-M-450-105 SH. 3, there are a quantity of 8 bolts installed on this plate. Due to the rigidity of the plate, the tension acting on the nuts and bolts is evenly distributed. As such, there is redundancy and margin in the case of a single nut should fail. However, this is not a likely possibility in the near term as the corrosion is minimal at this time.

Conclusion The CCW Head Tank 3T216 can meet its design basis functions, and the conditions described are not a personnel safety concern at this time.

However, changing the identified corroded bolts and installing weldment as required in the design drawing is recommended.
