- 13. Abstract of examinations and test. Include a list of examinations and tests and a statement concerning status of work required for the inspection plan.
IWL examinations were conducted in accordance with "The Concrete Containment lnservice Inspection Program for Turkey Point Unit 3 & 4". This program was written in accordance with ASME Section XI 2001 Edition with the 2003 Addenda and is subject to the limitations and modifications of 10 CFR 50.55a(b)(2) , 10 CFR 50.55a(g)(4) , and 10 CFR 50.55a(g)(6) . The requirements for the first inspection period as required by 10 CFR50.55a (g)(6) have been met as applicable to IWL, Program B of ASME Section XI. Precision Surveillance Corporation (PSC) supplied examination and testing services. See the attached report: Turkey Point Nuclear Plant- Unit 3 & 4, 45th Year Containment Tendon Surveillance for examination scope and results. This is the second ten-year interval for the IWE Program .
- 14. Abstract of Results of Examinations and Tests.
IWL Examinations Visual examinations were performed on the tendons and associated components , containment concrete and reinforcement steel. During the 45th year Containment tendon inspection , 3 tendons had loss of corrosion protection medium (grease) which exceeded the limit established in TS 4.6.1.6.1.e.2. AR2184466 identified tendons 12V22, 34V23, and 61V13 as having grease losses of 21 %, 17%, and 27%, respectively , compared to the allowed limit of 10% of the net tendon duct volume .
AR 2184991 identified a similar condition for tendons 12V11 , 56V28, and 61V20 which had grease losses of 34% , 27% , and 11 %, respectively.
- 15. Abstract of Corrective Measures Adequate grease volumes have been restored to the six tendons in accordance with approved vendor procedures and documented in Work Order 40448990. AR2184466-06 tracks inclusion of this condition in the final report

The results of this investigation are discussed in detail in the body of this report and are summarized in order of presentation below:
);;> Summary of Find ings:
- 1. SECTION 3.0 - SHEATHING FILLER ANALYSIS (SQ 7.0)
Water-soluble ion concentrations, moisture contents , and Neutralization (Base) Numbers of the tested sheathing fil ler samples were with in acceptable limits for all inspected tendons , per IWL-3221.4 .
- 2. SECTION 4.1 - GREASE CAP REMOVAL (SQ 6.0)
Acceptable grease coatings were found on all tendon ends inspected , and no unusual conditions were reported at any tendon end .
- 3. SECTION 4.2 - FREE WATER INSPECTION (SQ 6.1)
The presence of free water was observed at Unit 3 tendons 12V1 1 (6 oz .) 12V22 (4 oz.). Water Notification Letters were submitted to Turkey Point Engineering in response to each occurrence.
All other inspected tendon ends were found with no evidence of free water and are acceptable per IWL-3221 .3(e) .
- 4. SECTION 4.3 - ANCHORAGE INSPECTION (BUTTONHEAD COUNT) (SQ 8.0)
The gallery end of Unit 3 vertical tendon 12V01 was found with one (1) protruding buttonhead not previously reported . Non-conformance report (NCR) FN1130-001 was written by PSC Quality Control personnel , and Turkey Point Engineering was notified .
The buttress 5 end of Unit 4 hoop tendon 15H12 was found to have one (1 ) missing wire not previously reported. Non-conformance report (NCR) FN1130-009 was written by PSC Quality Control personnel ,
and Turkey Point Engineering was notified .
All other buttonheads at all inspected tendon ends were found in acceptable condition , per IWL-3221 .3(c).
- 5. SECTION 4.4 - ANCHORAGE INSPECTION (CORROSION LEVELS I CRACKS) (SQ 8.0)
No anchorage component cracks were observed on any inspected tendon end , and all inspected tendon anchorage components were found with acceptable corrosion levels, per IWL-3221 .3(a) and IWL-3221 .3(b) respectively .
- 6. SECTION 4.5 - BEARING PLATE CONCRETE INSPECTION (SQ 8.3)
All cracks observed with in the 24" perimeter of the concrete surrounding each inspected bearing plate were less than 0.01 O" in width , and are acceptable per IWL-3221.3(d).
- 7. SECTION 5.0- HYDRAULIC JACK CALIBRATIONS Two hydraulic jacks, 9191 and 9194 , are undergoing maintenance and have not completed the post-job calibration . The pre-job and post-job comparisons for these jacks are pending and will be included in the next revision of this report.
The remaining hydraulic jacks used for tendon lift-offs were calibrated before and after the surveillance period , and were found to be within the acceptable variation of +/-1.5%.

- 8. SECTION 6.0 - TENDON LIFT-OFF FORCES (SQ 9.0)
The average value of all tendon lift-off forces for a given tendon group exceeded the minimum required prestress value for their respective tendon group, per IWL-3221 .1(a) .
Unit 4 vertical tendon 34V05 had a lift-off value less than its pred icted lift-off value . After removal of the test wire and during retensioning , the tendon was restored to an acceptable force level.
All other tendons had an average lift-off force greater than its predicted lift-off value.
- 9. SECTION 7.0 - COMPARISON WITH ORIGINAL INST. DATA AND PREV. LOCK-OFFS In examining the average group percentage loss from the installation lock-off force values to the as-found lift-off force values during the 45th year surveillance , no abnormal losses were observed .
- 10. SECTION 8.0 - TENDON WIRE VISUAL INSPECTION & TENSILE TESTING (SQ 10.2 /SQ 10.3)
All wire samples, selected for testing per IWL-2523.1 , had acceptable corrosion level of 1 and were free of physical damage. All tendon wires removed and tested were found to have diameter values within the acceptable range and acceptably high yield stress and ultimate tensile strength.
- 11. SECTION 9.2 / 9.3 - TENDON RETENSIONING (FORCE and ELONGATION) (SQ 11 .0)
All detensioned tendons were retensioned to acceptable lock-off forces, per IWL-2523.3.
- 12. SECTION 9.4- TENDON RETENSIONING (BUTTONHEADS and WIRES) (SQ 11.0)
No broken, protruding , or detached buttonheads were recorded for any tendon due to restressing activities ,
and the anchorage condition at each tendon end was acceptable per IWL-3221 .3.
- 13. SECTION 10.1 / 10.2 - TENDON RESEALING (SQ 12.0 I SQ 12.1)
All tendon grease caps were properly installed to their respective tendon ends .
Three (3) Unit 3 scope tendons-12V11 (NCR-FN1130-007) , 12V22 (NCR-FN1130-002) , and 34V23 (NCR-FN1130-003) , three (3) Unit 3 gasket repair tendons-56V28 (NCR-FN1130-005) , 61V13 (NCR-FN1130-004), and 61V20 (NCR-FN1130-006), and two (2) Unit 4 gasket repair tendons-23V06 (NCR-FN1130-011 )
and 56V02 (NCR-FN 1130-010)-had a difference of the grease replaced , versus the grease lost, greater than 10% of the net duct volume for each respective tendon. The indicated Nonconformance Reports were written in response to each occurrence, and the conditions were evaluated by FPL Engineering .
All other tendon caps were refilled to acceptable levels, per IWL-3221.4 .
- 14. SECTION 11 .0-EXTERIOR CONCRETE SURFACE EXAMINATION (SQ 8.4)
The noted observations of the containment concrete General Visual Examination, including small bug holes, form grout patches , rust stains from embed rails, and shrinkage cracks, were recorded as "RI" (recordable indications) and "10" (information only) items. The condition of the exterior concrete surface was evaluated by FPL Engineering , per IWL-3211.

In summary, the Final Report for the 45th Year Tendon Surveillance at Turkey Point Nuclear Plant has concluded that the functional integrity of the selected post-tensioning system has met all the applicable code requirements, unless noted otherwise with non-conformance items, which are recorded , identified, and presented within this document and attached appendices, as required .

1.0 INTRODUCTION
This report details the 45th Year Turkey Point Nuclear Plant Unit 3 & 4 conta inment structure post tensioning systems IWL Tendon Surveillance. The surveillance program is a systematic means of assessing the quality and structural performance of the post-tension ing system.
1.1 The tendon surveillance program consists of a periodic inspection of the physical condition of a selected group of tendons . This program provides confidence in the condition and functional capability of the system , and an opportunity for timely corrective measures if adverse conditions are detected . The 45th Year Tendon Surveillance at the Turkey Point began on January 9, 2017, and on-site inspection was completed on April 6, 2017.
1.2 This surveillance period consisted of a visual inspection at Unit 3 and a physical inspection at Unit 4 of the conta inment buildings' post-tensioning system.
1.2.1 A visual tendon surveillance consists of sheathing filler inspection and testing , inspection for free water, anchorage inspection , concrete inspection around tendon bearing plates , and replacement of grease caps and grease after completion of all inspections.
1.2.2 A physical tendon surveillance consists of a visual inspection , plus force monitoring and inspection ,
as well as tensile testing , of removed wire samples.
1.3 The following documents define the specific requirements for selection of the inspection tendons, as well as the specific requ irements and acceptance criteria for the performance of inspected tendons for the 45th Year Tendon Surveillance at Turkey Point:

& Unit 4 45th Year Containment Building Tendon Surveillance - Precision Surveillance Corporation In-service Inspection Tendon Surveillance Program, Rev. 1 1.4 Sixteen (16) tendons were selected for the Unit 3 visual surveillance. Among the group of selected tendons , two (2) hoop tendons and two (2) vertical tendons had been affected by the RVCH repair and replacement activities, and were inspected in accordance with Table IWL-2521-2. The remaining twelve (12) tendons were selected in accordance with Section IWL-2521 and Table IWL-2521-1 of Ref. 3.
1.5 Sixteen (16) tendons were selected for the Unit 4 physical surveillance . Among the group of selected tendons, two (2) hoop tendons and two (2) vertical tendons had been affected by the RVCH repair and replacement activities, and were inspected in accordance with Table IWL-2521-2. The remaining twelve (12) tendons were selected in accordance with Section IWL-2521 and Table IWL-2521 -1 of Ref. 3.
1.5.1 Unit 4 hoop tendons 13H12, 15H34 , and 62H22 were determined to be inaccessible at one or both tendon ends. Per Ref. 3, a visual inspection was performed at the accessible tendon end (s) , and alternate tendons were selected . Tendons 13H16, 15H35, and 62H28 were selected as alternates for 13H12, 15H34, and 62H22, respectively . Therefore , a total of nineteen (19) tendons were inspected on Unit 4.
1.6 Eleven (11) tendons on Unit 3 and sixteen (16) on Unit 4 were selected by the plant for gasket repair, due to observable grease leaking at grease cap .
1.6.1 A protruding wire was observed at the gallery end of Unit 3 vertical tendon 12V01 . Under the direction of the plant, a grease sample was collected for testing (Ref. 7, SQ 7.0) and an inspection of the anchorage components was conducted (Ref. 7, SQ 8.0) . All other gasket repair tendons were inspected based on the criteria in Table 1-2 and Table 1-4.

The surveillance procedural steps are summarized below:
2.1 Removal of tendon grease cap and visual examination of sheathing filler (grease) .
2.2 Analytical testing of sheathing filler (grease) samples.
2.3 Examination for the presence of free water and analysis of water samples (if applicable).
2.4 Inspection of the anchorage assembly of each surveillance tendon end for deleterious conditions such as corrosion , cracks , and protruding , broken or missing wires or buttonheads.
2.5 Inspection of 24" of concrete surrounding each surveillance tendon bearing plate for any recordable indications.
2.6 Measurement of the lift-off force for each physical surveillance tendon .
2.7 Detensioning of tendons selected for wire removal (if applicable) .
2.8 Removal of test wire from designated physical surveillance tendon for examination and testing (if applicable) .
2.9 Visual inspection for corrosion , pitting, or any significant physical change of the removed wires (if applicable) .
2.10 Testing of samples taken from wires removed from tendons for yield strength , ultimate strength, and percentage elongation at failure (if applicable).
2.11 Retensioning of detensioned tendons and measuring the corresponding tendon elongation (if applicable).
2.12 Resealing tendon caps and replacement of lost sheathing filler (grease).
2.13 General visual inspection of the accessible exterior concrete surfaces of the containment buildings.
2.14 Evaluation oftest and inspection results to assess the general condition of the post-tensioning system.

Table IWL-2525-1 specifies the limits of the allowable water content, water-soluble ion amounts, and the base neutralization number.
3.2 Two (2) samples of grease were removed from each end of the surveillance tendons, each one (1) quart in volume . Chemical tests were performed on all collected samples by Suburban Laboratories , Inc.
Sheathing filler testing was done per IWL-2525.2 requirements .
3.3 Reserve alkalinity (Base) number limit is in accordance with Note (4) from Table IWL-2525-1 quoted below:
Table IWL-2525-1 Note (4): "The base number shall be at least 50% of the as-installed value, unless the as-installed value is 5 or less, in which case the base number shall be no less than zero. If the tendon duct is filled with a mixture of materials having various as-installed base numbers, the lowest number shall govern acceptance."
3.3.1 The requ ired base number for the Turkey Point Unit 3 and Unit 4 are 0 mg KOH I g SAMPLE per PSC Procedure SQ7.0, Sec. 8.0.
3.4 Laboratory analysis results of the sheathing filler are as follows:
)> ION CONCENTRATION -All tendon grease samples' ion concentrations were within their respective acceptable limits.
)> MOISTURE CONTENT - All tendon grease samples' moisture contents (measured in percent of dry weight) were within the acceptable limit.
)> RESERVE ALKALINITY (BASE) NUMBER - All tendon grease samples' base numbers were above the specified acceptance limit.
3.5 No signs of degradation of the grease, by either color or consistency , were noted on any of the tendons tested.
3.6 Laboratory analysis results are contained in Appendix B, and the results , along with the acceptance limits, are summarized in Table 3-1 through Table 3-3. In Table 3-1 through Table 3-3, "N/A" indicates no inspection was performed at that tendon end.

In the following discussion , all procedures referred to are outlined in Appendix E (Ref. 7) of this report, and all completed data sheets are included in Appendix A.
4.1 SQ 6.0- GREASE CAP REMOVAL 4.1.1 Inspection of the anchorage components began by removing the grease cap from each end of the inspected tendons, per PSC Procedure SQ 6.0.
)> Acceptable grease coatings were found on all tendon ends inspected , and no unusual conditions were reported at any tendon end.
4.1.2 The observed percent of grease coating for each anchorage component is contained in SQ 6.0 data sheets of Appendix A and is tabulated in Table 4-1 through Table 4-5.

4.2.2 According to ASME,Section XI , Subsection IWL-3221.3(e), "(t)he condition of tendon anchorage areas is acceptable if ... there is no evidence of free water."
);> The presence of free water was observed at Unit 3 tendons 12V11 (6 oz.) and 12V22 (4 oz .).
Water Notification Letters were submitted to Turkey Point Engineering in response to each occurrence.
);> All other inspected tendon ends were found with no evidence of free water and are acceptable per IWL-3221.3(e).
4.2.3 Samples of the observed water were collected at Unit 3 tendons 12V11 and 12V22, per PSC Procedure SQ6.1 . During the process of being released by the plant, RP personnel inadvertently discarded the samples. The details of the missing/discarded samples are recorded in PTN AR#2187886 (Appendix K) .
4.2.4 Inspection results are contained in SQ 6.1 data sheets of Appendix A and are summarized in Table 4-6 through Table 4-9. In Table 4-6 through Table 4-9, "N/A" means that the surveillance item did not apply to the tendon end.

Missing, broken , damaged, unseated , or improperly formed (double) buttonheads were identified on the surveillance data sheet's schematic of the anchorage.
Buttonhead inspection was done per PSC Procedure SQ 8.0.
4.3.2 According to ASME ,Section XI , Subsection IWL-3221.3(c), "[t]he condition of tendon anchorage areas is acceptable if... broken or unseated wires , broken strands , and detached buttonheads were documented and accepted during a preservice examination or during a previous inservice examination ."
J;>. The gallery end of Unit 3 vertical tendon 12V01 was found with one (1) protruding button head not previously reported . Non-conformance report (NCR) FN1130-001 was written by PSC Quality Control personnel , and Turkey Point Engineering was notified .
J;>. The buttress 5 end of Unit 4 hoop tendon 15H 12 was found to have one (1) missing wire not previously reported . Non-conformance report (NCR) FN1130-009 was written by PSC Quality Control personnel, and Turkey Point Engineering was notified .
J;>. All other buttonheads at all inspected tendon ends were found in acceptable condition, per IWL-3221 .3(c).
4.3.3 Inspection results are contained in SQ 8.0 data sheets of Appendix A and are summarized in Table 4-10 through Table 4-12. In Table 4-10 through Table 4-12, "N/A" means that the surveillance item did not apply to the tendon end .

sig ns of excessive stress, and cracking . Tendon wire buttonheads were also inspected for corrosion and cracking . Corrosion levels on all listed anchorage components were quantified accord ing to PSC Procedure SQ 8.0.
4.4.2 According to ASME,Section XI , Subsection IWL-3221 .3(a) and IWL-3221 .3(b) , "[t] he condition of tendon anchorage areas is accepta ble if ... (a) there is no evidence of cracking in anchor heads, shims, or bearin g plates .. . [and] (b) there is no evidence of active corrosion ."
- .. No evidence of cracking was observed on any of the anchorage components of any tendon end, and all inspected tendon anchorage components were found with acceptable corrosion levels.

The purpose of this inspection is to detect any cracks in the concrete greater than 0.010" in width .
4.5.2 According to ASME ,Section XI , Subsection IWL-3221 .3(d), "[t]he condition of tendon anchorage areas is acceptable if.. . cracks in the concrete adjacent to the bearing plates do not exceed 0.01 in .

This is a process where a straight line is mathematically best fit to a set of data points (in th is case , force versus the gauge pressure). This results in a linear equation which relates the ram area (slope) and the constant (y-intercept) for each jack calibration , allowing the conversion of pressure to force and vice versa .
5.1 A before and after comparison of the stressing rams used during the surveillance was performed .
- .. Hydraulic jacks 9191 and 9194 are undergoing maintenance and have not completed the post-job calibration . The pre-job and post-job comparisons for these jacks are pending and will be included in the next revision of this report.
- .. The comparison of the remaining jacks revealed a rang e of variations from 0.198% to 0.378%,
indicating that all jacks remained in a properly calibrated status throughout the surveillance .
Acceptab le variation is +/-1 .5%.
5.2 Complete calibration data for the hydrau lic jacks used during the tendon surveillance is contained in Appendix D and is summarized in Table 5-1.

First, measurements are taken of the anchorage threads in order to assign an appropriate stressing adaptor to each physically inspected tendon . Second , lift-offs are performed on the physically inspected tendons , and the measurements are recorded .
6.2 SQ 9.0 - TENDON LIFT-OFF FORCES Lift-off is defined as the jacking force required for transferring the load on a tendon from the shim stack to the stressing ram, and is representative of the force held by that tendon . A lift-off is performed on each physical surveillance tendon in order to monitor the force exerted by the tendon onto the containment structure. Obtaining lift-off force measurements for inspected tendons was done in accordance with IWL-2522 and PSC Procedure SQ 9.0.
6.2.1 According to ASME ,Section XI , Subsection IWL-3221 .1(a) , "[t]endon forces ... are acceptable if...
[t]he average of all measured tendon forces , including those measured in IWL-3221 .1(b)(2), for each type of tendon is equal to or greater than the minimum requ ired prestress specified at the anchorage for that type of tendon ."
Jo>- The average lift-off value of each type of tendon (Domes , Hoops, Verticals) , including tendons from IWL-3221 .1(b)(2), exceeded the minimum required group average prestress value for that type of tendon .
6.2.2 According to ASME ,Section XI , Subsection IWL-3221 .1(b) , "[t]endon forces ... are acceptable if...
[t]he measured force in each tendon is not less than 95% of the predicted force ... "
Jo>- The lift-off force measured in all tendons exceeded the 95% of the predicted force lift-off value fo r the 45th Year Surveillance period.
6.2.3 Because all tendons had an average lift-off greater than 95% of the tendon's respective predicted lift-off value , the conditional items in section IWL-3221 .1(b) do not apply.
6.2.4 Lift-off force measurements are contained in SQ 9.0 data sheets of Appendix A and are summarized in Table 6-1 through Table 6-3. In Table 6-1 through Table 6-3, "N/A" means that the tendon was not stressed at that end .
6.2.5 Upon completion of the lift-off, a visual verification was performed to identify any changes in the condition of the tendon end.
Ji>- No protruding or broken wires or damage to any anchorage components was noted during or immediately following the lift-offs.

The lift-off forces are compared in order to detect any abnormal force loss that would possibly indicate an underestimation of the creep , shrinkage , and/or elastic shortening effects in the containment building.
)> Based on the comparison results , tendon force losses are within the anticipated range , and no abnormal lift-off force loss averaged over surveillance tendons from each group is noted .
7.2 Tendon lock-off forces for each tendon were obta ined from the Unit 3 and Unit 4 installation cards, either at original installation or during RVCH project for the respective tendons .
7.3 Lift-off force values for all inspected tendons are contained in SQ 9.0 data sheets of Appendix A. Force comparison data and force losses are summarized in Table 7-1 through Table 7-3.
7.4 Force loss percentage is calculated from the equation below, where Fl..OCMIFF represents the tendon lock-off force at original installation or restressing, and fLFT-<lFF represents the tendon as-found lift-off force during the surveillance.
7.5 Comparison results provided in this section are for information only. There are no established acceptance criteria for percentage of stressing loss. The losses appear to be as expected when compared to other containments of this style and age.

The tendon wires were sent to Exova testing laboratory. Each wire segment was inspected for corrosion , yield strength , ultimate tensile strength, and percent elongation in accordance with IWL-2523.2.
Wire testing was done per ASTM A421 for the yield strength and per ASTM A370-09a for the elongation .
8.2 According to ASME,Section XI, Subsection IWL-3221 .2(a) , "[t]he condition of wire or strand samples is acceptable if... samples are free of physical damage. "
~ All wire samples , selected for testing per IWL-2523.1, had acceptable corrosion level of "A" and were free of physical damage.
8.3 According to ASME ,Section XI , Subsection IWL-3221 .2(b) , "[t]he condition of wire or strand samples is acceptable if ... sample ultimate tensile strength and elongation are not less than minimum specified values ."
~ ULTl MATE TENSILE STRENGTH - The ultimate tensile strength for all wire samples exceeded the minimum specified value of 240 ksi. The lowest recorded ultimate tensile strength was 271 .5 ksi .
~ ELONGATION - The percent elongation at sample failure was at or above the required minimum of 4.0% for all samples tested . The recorded elongation values on the samples varied from 4.3% to 6.8%.
8.4 Wire diameter and yield strength test results were as follows :
~ WIRE DIAMETER - All tendon wire sample diameter measurements were within the acceptable range of 0.250" +/- 0.002".
~ YIELD STRENGTH - The yield strength for all wire samples exceeded the min imum of 204 ksi. The lowest recorded yield strength was 216.1 ksi .
8.5 Wire test results data are contained in Appendix C and summarized in Table 8-1 and Table 8-2.

In the process of retensioning , observations were recorded on tendon lock-off forces , tendon wire elongations , and the post-retensioning condition of tendon wires and button heads.
9.2 SQ 11 .0 - TENDON LOCK-OFF FORCES 9.2.1 According to PSC Procedure SQ 11 .0 (Ref. 7, Sec. 9.9.3) , for tendons whose lift-off force value during the surveillance is greater than or equal to the predicted lift-off value for that surveillance, the lock-off force shall be within -0/+6% of the as-found lift-off force.
- .. Unit 4 vertical tendon 56V29 had an average as-found lift-off value greater than 70% of the minimum ultimate tensile strength. Per PSC Procedure SQ 11 .0, Section 9.9.3 and Data Sheet SQ11.0, the target restressing lock-off force was within 67% to 70% of the minimum ultimate tensile strength, instead of -0/+6% of the as-found lift-off force .
- .. The as-found lift-off forces for all other tendons exceeded their respective specified limits for all tendons designated for de/retensioning due to wire removal and were restored to acceptable force levels.
9.2.2 For any tendon whose lift-off force value during the surveillance is less than the predicted lift-off value for that surveillance , retensioning is required per PSC Procedure SQ 9.0 (Ref. 7, Sec. 10.5) ,
and the lock-off force shall be within -0/+6% of the predicted lift-off force of that tendon.
- .. Unit 4 vertical tendon 34V05 had a lift-off value less than its predicted lift-off value . After removal of the test wire and during retensioning , the tendon was restored to an acceptable force level.
9.2.3 According to ASME ,Section XI, Subsection IWL-2523.3(a) , " [~endons that have been detensioned shall be re tensioned to at least the force predicted for the tendon at the time of the test. However, the tendon force after retensioning shall not exceed 70% of the specified minimum ultimate tensile strength of the tendon based on the number of effective wires or strands in the tendon at the time of retensioning."
- .. The lock-off force values of all retensioned tendons were within acceptable levels, and the lock-off force values were less than their respective maximum specified value of 70% of the minimum ultimate tensile strength.
9.2.4 As-found lift-off and as-left lock-off force values are contained in SQ 9.0 and SQ 11.0 data sheets of Appendix A, respectively. Predicted lift-off force values for each tendon of the surveillance scope are summarized in Table 2-1 and Table 2-2 of the PSC Surveillance Procedure SQ 9.1 .
9.2 .5 Comparison of tendon lock-off force to the applicable (as-found I predicted) lift-off force variation percent interval is summarized in Table 9-1 and Table 9-2. In Table 9-1 and Table 9-2, "N/A" means that the tendon was not stressed on that end .
9.2.6 Measured variation percentage for the retensioning force is calculated from the equation below, where F~ represents the tendon lock-off force after retensioning , and FLFT.oFF represents (a) the tendon as-found lift-off force in case of tendons whose as-found lift-off force values exceeded their predicted lift-off force values , and (b) the tendon predicted lift-off force otherwise.

l'> After thread strength verifications of Unit 4 tendons 1D45, 13H 16, and 34V05, it was determined the threads strength would be insufficient to achieve Overstress at 80% of the tendon capacity . The overstress force of tendons 1D45, 13H 16, and 34V05 was calculated at 70% of the Guaranteed Ultimate Tensile Strength (GUTS). The recalculation of the overstress data is addressed in Field Change Requests FCR-N1130-002, FCR-N1130-003 , and FCR-N1130-004.
l'> Based on the thread measurements on Unit 4 vertical tendon 34V05, it was determined the threads maximum capacity of 689 kips , which is less than 70% of GUTS (742 kips) . This was evaluated by FPL Engineering, and maximum overstress value for 34V05 was reduced to 689 kips (approximately 65% GUTS) . The corresponding elongation at that overstress force was calculated and included in FCR-N1130-005.
l'> The elongation values observed at 1D45, 13H16, and 34V05 were acceptable when compared to the factored original elongation value .
l'> All other elongation values recorded during the retensioning of tendons were compared to elongation values recorded at the time of last measurement (which was in this case the time of original installation). The variation percentage between respective elongation values was found to be acceptable for all retensioned tendons.
9.3.2 Elongation values for de/retensioned tendons during the 45th Year Surveillance are contained in SQ 11 .0 data sheets of Appendix A. Elongation values during the previous measurement (tendon installation) are obtained from installation stressing data, either at original installation or during the RVCH Project for the respective tendons .
9.3.3 Comparison of tendon elongation values during retensioning to elongation values during installation are summarized in Table 9-1 and Table 9-2. In Table 9-1 and Table 9-2, "N/A" means that the tendon was not stressed at that end.

Tendon resealing consists of the grease cap replacement and the sheathing filler (grease) replacement.
10.1 SQ 12.0 GREASE CAP REPLACEMENT 10.1.1 After the tendons were locked-off, all anchorage components were hand-coated with cold grease to ensure complete coverage . The caps were then anchored back to their respective tendon ends per PSC Procedure SQ 12.0.
- l> All tendon grease caps were properly installed to their respective tendon ends using new gaskets.
10.1 .2 Grease cap replacement checklist items for all tendons are contained in SQ 12.0 data sheets of Appendix A.
10.2 SQ 12.1 GREASE REPLACEMENT Upon acceptable grease cap replacement, the necessary amount of sheathing filler was poured , hand-pumped, or pressure-pumped (as required) back into the caps through cap inlets per PSC Procedure SQ 12.1. The tendon cap inlet plug was then installed , and any excess grease was cleaned from the outer cap surfaces and any surrounding concrete areas.
10.2.1 According to ASME ,Section XI , Subsection IWL-3221.4, " [~he absolute difference between the amount of[corrosion protection medium (grease)] removed and the amount replaced shall not exceed 10% of the tendon net duct volume. "
- l> Three (3) Unit 3 scope tendons-12V11 (NCR-FN1130-007), 12V22 (NCR-FN1130-002), and 34V23 (NCR-FN1130-003) , three (3) Unit 3 gasket repair tendons-56V28 (NCR-FN1130-005) ,
61V13 (NCR-FN1130-004), and 61 V20 (NCR-FN 1130-006), and two (2) Unit 4 gasket repair tendons-23V06 (NCR-FN1130-011) and 56V02 (NCR-FN1130-010)-had a difference of the grease replaced , versus the grease lost, greater than 10% of the net duct volume for each respective tendon . The indicated Nonconformance Reports were written in response to each occurrence, and the conditions were evaluated by FPL Engineering .
- l> All other tendon caps were refilled within the acceptable limits.
10.2.2 Grease replacement data is contained in SQ 12.1 data sheets of Appendix A and is summarized in Table 10-1 through Table 10-5. In Table 10-1 through Table 10-5, "NIA" means that no grease was removed or replaced at that tendon end.

The VT-3 examinations were performed in accordance with PSC Procedure SQ 8.4 .
11.2 According to ASME ,Section XI , Subsection IWL-3211 , "[t]he conditi on of the concrete surface and tendon end anchorage areas is acceptable if the Responsible Engineer determines that there is no evidence of damage or degradation , corrosion protection medium leakage, or end-cap deformation requiring further evaluation or performance of repair/replacement activities ."
11.3 Visual inspection results, as well as the Responsible Engineer's assessment, for the containment buildings' exterior concrete surface are contained in Appendix J.

12.0 CONCLUSION
12.1 A review of this surveillance was conducted per IWL-3221 Unbonded Post-Tensioning Systems and is outlined below:
IWL-3221 Acceptance by Examination IWL-3221.1 Tendon Force and Elongation. Tendon forces and elongation are acceptable if the following conditions are met:
(a) The average of all measured tendon forces, including those measured in IWL-3221 .1(b)(2), for each type of tendon is equal to or greater than the minimum required prestress specified at the anchorage for that type of tendon .
Results: The average of all measured tendon forces was above the minimum required prestress force . The average of each "Original Installation" group's lift-off values were as follows: Unit 4 domes - 590.38 kips, Unit 4 hoops - 639.93 kips, Unit 4 verticals - 672.62 kips. These values exceeded the minimum prestress forces for domes (531 .00 kips) , hoops (491.60 kips) , and verticals (522.00 kips) .
The average of each RVCH group's lift-off values were as follows : Unit 3 RVCH hoops -607.70 kips and Unit 3 RVCH verticals - 727.12 kips ; Unit 4 RVCH hoops
- 608 .91 kips and Unit 3 RVCH verticals - 721 .66 kips. These values exceeded the minimum design prestress force for hoops (491.60 kips) and verticals (522.00 kips) .
(b) The measured force in each individual tendon is not less than 95% of the predicted force unless the following conditions are satisfied.
(1) The measured force in not more than one tendon is between 90% and 95% of the predicted force .
(2) The measured forces in two tendons located adjacent to the tendon in IWL-3221 .1(b)(1) are not less than 95% of the predicted forces.
(3) For tendons requiring augmented examination in accordance with Table IWL-2521-2, Item L2-10, the measured forces in two like tendons located nearest to but on opposite sides of the tendon described in IWL 3221-1(b)(1) are not less than 95% of the predicted forces.
(4) The measured forces in all the remaining sample tendons are not less than 95% of the predicted force .
Results: All of the tendon lift-off forces were found within the acceptable limits. All tendons were found with an average lift-off value greater than 95% of the tendon predicted force.
IWL-3221.2 Tendon Wire or Strand Samples. The condition of wire or strand samples is acceptable if-(a) samples are free of physical damage; Results: All test wire samples collected for this inspection were found to have acceptable corrosion levels and free of physical damage.
(b) sample ultimate tensile strength and elongation are not Jess than minimum specified values.
Results : All test wire samples had acceptable results for ultimate tensile stress
(<:240ksi) and elongation (<:4.0%).
IWL-3221.3 Tendon Anchorage Areas. The condition of tendon anchorage areas is acceptable if-(a) there is no evidence of cracking in anchor heads, shims, or bearing plates; Results: Detailed inspections did not reveal any cracks in the anchorage components for any inspected tendon end.

(c) broken or unseated wires, broken strands, and detached buttonheads were documented and accepted during a pre-service examination or during a previous in-service examination; Results: The gallery end of Unit 3 vertical tendon 12V01 was found with one (1) protruding button head not previously reported . Non-conformance report (NCR) FN 1130-001 was written by PSC Quality Control personnel , and Turkey Point Engineering was notified .
The buttress 5 end of Unit 4 hoop tendon 1SH12 was found to have one (1) missing wire not previously reported . Non-conformance report (NCR) FN 1130-009 was written by PSC Quality Control personnel , and Turkey Point Engineering was notified.
All other buttonheads at all inspected tendon ends were found in acceptable condition, per IWL-3221 .3(c) .
(d) cracks in the concrete adjacent to the bearing plates do not exceed 0.01 in .(3mm) in width; Results: No cracks exceeding 0.010" were detected in the 24" of concrete adjacent to the bearing plates of the tendon ends inspected .
(e) there is no evidence of free water.
Results: The presence of free water was observed at Unit 3 tendons 12V11 (6 oz.) 12V22 (4 oz.). Water Notification Letters were submitted to Turkey Point Engineering in response to each occurrence. No other free water was observed at any other tendon end inspected .
IWL-3221 .4 Corrosion Protection Medium. Corrosion protection medium is acceptable when the reserve alkalinity, water content and soluble ion concentrations of all samples are within the limits specified in Table IWL-2525-1 . The absolute difference between the amount removed and the amount replaced shall not exceed 10% of the tendon net duct volume.
Results: Reserve alkalinity (base number), water-soluble ion concentrations, moisture contents of the tested sheathing filler samples were within acceptable limits for all inspected tendons, per IWL-3221.4.
Three (3) Unit 3 scope tendons-12V11 (NCR-FN1130-007) , 12V22 (NCR-FN1130-002), and 34V23 (NCR-FN1130-003) , three (3) Unit 3 gasket repair tendons-56V28 (NCR-FN1130-005), 61V13 (NCR-FN1130-004) , and 61V20 (NCR-FN 1130-006), and two (2) Unit 4 gasket repair tendons-23V06 (NCR-FN 1130-011) and 56V02 (NCR-FN 1130-010)-had a difference of the grease replaced , versus the grease lost, greater than 10% of the net duct volume for each respective tendon . The indicated Nonconformance Reports were written in response to each occurrence, and the conditions were evaluated by FPL Engineering. No other tendon accepted more than 10% of its net duct volume more than was removed , and all refills were acceptable.
Based upon the evaluation of the In-Service Inspection results for the Turkey Point Unit 3 and Unit 4 45th Year Containment Building Tendon Surveillance reported herein, PSC concludes that the containment structure has experienced no abnormal degradation of the post-tensioning system and all nonconformance items have been identified, documented, and reported as required . *
