Dear Mr. Yachimiak Jr.:

In response to your letter dated April 10, 1986 concerning inadequate thread engagement on motor hold down bolts for service water systems pump / motor, 2 SWP P1 A-F, located at Niagara Mohawk Power Corporation, Nine Mile Point -

Unit 2, please be advised as follows:

Findings:

The concern was: is there adequate thread engagement in motor hold down bolts for 2 SWP & Pl? The measurements of the bolts, the washer, the motor foot, the shim and base were taken, then compared to see if actual thread engagement was more than min. allowable. The conclusion was that there is insufficient thread engagement in the motor hold down bolts.

Investigative Action:

An unauthorized field change replaced the shipped lock washer with a lock washer or shim which considerably exceed design dimensions and substantially reduced thread engagement. (see attachment) Stone & Webster express concerns of improper thread engagement to Goulds because of this unauthorized change.

Goulds recommendation was to change bolts to 2i" in length. Since this was a specific customer initiated change, no further action was necessary with other customers.
