Incorrectly Stamped Nameplates-on Certain ASCO Nuclear Qualified Valves

Dear Mr. Murley:

We enclose information relating to certain valves manufactured by Automatic Switch Co. ("ASCO").

As you will see from the enclosed materials, it nas been determined that the nameplates of these valves may have been stamped with the incorrect rebuild kit number.

This condition may result in the valve being improperly rebuilt if an incorrect rebuild kit is utilized.

A complete list of any potentially affected valves is attached.

Our records have revealed only two instances, as detailed in the attached materials, in which an incorrect rebuild kit was supplied.

ASCO does not have adequate knowledge of the actual installation and operating conditions of these valves to determine whether their malfunction would create a " substantial safety hazard" as defined in 30CFR21.3 so that formal notification would be required by 10CFR21.21.

We are likewise unable to conduct the evaluation necessary to make such a determination.

Although ASCO is not obliged to submit formal notification pursuant to 20CFR21, we are reporting this information to you consistent with the provisions of 30CFR21.2 utilizing the relevant portions of the format set forth at 10CFR21.23.

In addition, we are taking action necessary to correct product in the field as well as to insure that this problem does not recur.

ASCO will certainly cooperate with any further investigation of this problem that may be required.

Nature of the failure and potential safety hazard:

ASCO has, for approximately 5 years, stamped rebuild kit numbers on valve namep)ates.

We have discovered that, due to a clerical error, the nameplates of some of these valves may have been stamped with the incorrect rebuild kit numbers as follows:

a.

Rebuild kit number for a normally closed valve stamped on the nameplate of a normally open valve.

Installation of the incorrect kit in this situation could result in leakage to exhaust in the deenergized position.

The exhaust port in a normally open valve is Port #2 (refer to enclosed drawing JVA-206-380).

Since the upper spring (Item 13) in a normally closed valve (and rebuild i

kit) is not as strong as that used in a normally open valve, installation of the normally closed upper spring may, depending upon the pressure applied at Port

  1. 3, result in insufficient force to maintain - the upper disc (Item 14) in its seated position when the valve is de-energized.

Leakage past the upper seat (out Port #2, exhaust) will result in a reduction in pressure at the cylinder port l

(Port #1).

This, in turn, would result in an increase in the response time of the device being controlled (control valve, cylinder, etc.).

b.

Rebuild kit number for a normally closed valve stamped on the nameplate of a universal valve.

A Universal type valve is designed to be operated as either a normally open or normally closed, depending on the piping configuration.

Installation of the incorrect kit in this situation would have no effect if the valve was being used in the normally closed mode.

However, if the valve was used in the normally open mode, the valve would leak to exhaust in the deenergized position.

the same reason, and would have the same effect, as F

decribed in 3a above, i.e.,

depending. upon the pressure applied at Port

  1. 3, there may be insufficient spring force to maintain the upper disc in the seated position, resulting. in leak'ge out Port
  1. 2 and a reduction in a

pressure at the cylinder port.

c.

Rebuild kit number for a valve having a 3/16" diameter seat stamped on the nameplate of a valve having a 1/4" diameter seat.

Installation of the incorrect rebuild kit in this situation would result in a reduction in flow.

L d.

Rebuild kit number for a valve without a manual operator stamped on the - nameplate of a valve provided with a manual operator-(suffix "M0").

The only difference between a rebuild kit for a valve with a manual operator and a rebuild kit for the same valve without a manual operator is that the kit for the valve with the manual operator includes one additional gasket.

This ' gasket is shown on.the enclosed ASCO drawing JVA-206-380, and is identified as Item.27C (gasket, MO).

This gasket is an external seal and does not affect the operational characteristics of the valve.

Upon installation of the incorrect (non "MO") kit, it may be presumed that the original gasket (Item 27C) would be re-used.

However, should this gasket be removed and not re-installed upon re-assembly of the

valve, slight external leakage past the manual operator stuffing box (Item 27D on drawing JVA-206-380) threads may occur, which would have no affect on valve operation.

ASCO has researched its records to determine to whom affected valves have been shipped.

We have also determined, by a review of our sales records that, with two exceptions, none of the facilities to whom the affected valves have been shipped, have purchased incorrect kits for these valves.

Exceptions are as follows:

a.

The sale of four pieces, rebuild kit 210-098 (which is the rebuild kit for a normally closed valve),

for a Universal j

valve.

This sale was made through our distributor, Ives Equipment Co. to New Hampshire Yankee, Seabrook Station, and shipped on January 7, 1986.

b.

The sale of 3 pcs. rebuild kit 210-169 (which is the rebuild l

kit for a vlave without a manual operator) for a valve with a manual operator.

This sales was made through our distributor, Brownell Electro, Inc. to Georgia Power Co. and shipped on October 5, 1982.
