Description of the Problem  !

The vendor's cable supplied with Fluid Component Model FR-72-4R flow element / switches 2HVR*FE/FS 18A and C was. not qualified for Category I application. There also was no technical data for the cable provided.

The vendor also supplied flow switches 2HVR*FE/FS 18B and D with cables of insufficient length. No vendor-supplied cable was found for switches 2HVR*FS36A and B and 2HVR*FS37A and B.

Since the switches 18A, B, C, and D had cables which were unacceptable, it is presumed, for the pp poses of evaluation, that had the problem not been detected, switches 36A, 36B, and 37A, 37B would use the same type of unquali-fied vendor-supplied cable as was supplied with switches 18A, B, C, and D.

Use of improper cable could result in cable failure which could cause false signals from these flow switches.

Analysis of Safety Implications Cable failure involving switches 2HVR*FS36A/37A (Division I Control ) and 2HVR*FS36B/378 (Division II Control ) could involve common failure of both switches in both divisions resulting in incorrect (false) " normal flow exists" control signals. This could prevent the automatic start function of the standby gas treatment system and the reactor building emergency ventilation unit coolers 2HVR*UC413A and B.

i Combinations of failures of the affected flow switches could prevent the reactor building emergency ventilation unit coolers 2HVR*UC413A and B from starting.

If this were to occuc during a loss of coolant accident event in the primary containment, the secondary containment pressure could remain at a pressure above -0.25 in, water gauge for a time longer than analyzed and could result in higher offsite doses. If the above described condition had remained uncorrected, it could have adversely affected the safety of operation of the plant.

Corrective Action Repair For flow switches 2HVR*FE/FS 18A and 18C, Stone & Webster has replaced the unqualified vendor-supplied cables with NJP-14 type cable which meets or exceeds all qualification requirements, as documented in Nonconformance and Disposition Report #15,029.

For units 2HVR*FE/FS 188 and 18D, which were supplied with cables of insufficient length, the cables have been replaced with qualified Category I cable as documented in Advance Change Notice No. 0046310.

Units 2HVR*FE/FS 36A, 363, 37A, and 37B were delivered to the site without vendor cable and were installed with qualified Category I cable.
