V NIAGARA M0 HAWK POWER CORPORATION NINE MILE POINT - UNIT 2 DOCKET NO. 50-410 Final Report for a Problem Concerning Borg Warner Electro Hydraulic Actuator 011 Leak On The D. C. Motor (55(e) 09)
Description of the Problem The problem concerns leakage of hydraulic fluid on the D.
C. motor of one actuator described in Specification NMP2 C051M.
This actuator was the only one filled with hydraulic fluid when the leak was detected.
Inspection of this actuator showed significant leakage from the pump seal, allowing oil to saturate the D.C. motor.
The oil on the D.C. motor could cause malfunction of the unit.
The actuator problem affects valves 2HVK*TV21A and B, 22A and B, 2ICS*PCVil5, 2SWP*TV35A and B which were procured undEr Specification NMP2 C051M and which are located in the control building chilled water, reactor core isolation ct,oling and service water systems.
Other valves procured f rom Clow Corporation under Specification NMP2 P304Y which also have Borg Warner actuators are a different design and therefore, are not subject to this problem.
Analysis of Safety Implications 011 saturation of the D.
C. motors on the above mentioned actuators could result in improper operation of their associated system which, if uncorrected, could have adversely affected the safety of operations of the plant.
Corrective Action The seven affected valves mentioned above have been returned to the manufacturer and have been rebuilt to include new seals and qualified PA0 hydraulic fluid.
The subject valves have been re-installed with the motor-pump units in the actuators reoriented.
