This preliminary notification constitutes EARLY notice of an event of x
3 POSSIBLE safety or public interest significance. The information
/ presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.
Facility: Indian Point Unit No. 3 (DN-286) 23 Subj ect: DEFECTIVE WELD ON BORON INJECTION TANK SAFE END While shutdown for a scheduled maintenance outage, a minor (weeping) leak was observed in the vicinity of a factory-made weld on the outlet nozzle of the boron injection tank. Subsequent visual inspections and radiographs confirmed that the weld joining the pipe safe-end to the tank was defective. The defect was apparently caused by lack of weld material. A weld section is being examined in the licensee's metallurgy laboratory. The licensee has determined that a similar factory-made weld on the inlet nozzle was acceptable.
The safe-end will be removed and rewelded. This is expected to delay the scheduled startup by 7 to 10 days. The licensee is also determining if he has other tanks installed that were made by the same vendor (Joseph Oat and Sons, Incorporated). Region I is reviewing the generic aspects of this problem.
Media interest is expected because of close attention given to opera-tions at Indian Point. The State of New York has been informed. A press release is not planned.
Region I, NRC, Philadelphia, Pennsylvania received notification of this occurrence by telephone from the licensee's Director of Technical Engineer-ing at 3: 30 p.m. , September 21, 1976. The information is current as of 8:30 a.m., September 22, 1976.
