I '. g APPENDIX A NOI' ICE OF VIOIATION Power Authority of the State Docket No. 50-286 of New York License No. DPR-64 Indien Point 3 Nuclear Power Plant As a result of the inspection conducted on March 14 - 25, 1989, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C, Supplement IV) the following violation was identified:

Technical Specifications, section 6.12, "High Radiation Area", states that any individual permitted to enter high radiation areas shall be provided or amnied by one or more of the following:

a. A radiation monitoring device which continuously iMicates the radiation i dose rate in the area,

b. A radiation monitoring device which continuously integrates the radiation dose rate in the area ard alarms when a preset integrated dose is received. Entry jnto such areas with this monitoring device may be made after the dose rate level in the area has been established and personnel have been made knowledgeable of them.

c. An individual qualified in radiation protection promdures who is equipped with a radiation dose rate monitoring device. 'Ihis individual shall be responsible for providing positive cx>ntrol over the activities within the area and shall perform periodic radiation surveillance at the frequency specified by the facility Health mysicist in the Radiation Work Permit.

Contrary to the above, during the period March 14 - 25, 1989, ard for an undetermined time before that period, workers were allowed to enter high radiation areas without being provided or ame_nied by one of the options specified in the Technical Specifications, as listed above.

'Ihis is a Severity level IV violation.

Pursuant to the provisions of 10 CFR 2.201, New York Power Authority is hereby to submit to this office within thirty days of the date of the letter ch transmitted this notice, a written statm ent or explanation in reply, including: (1) the corrective which have been taken and the results achieved; ch will be taken to avoid further violations;(2)and corrective steps (3) the date when full conpliance will be achieved. Where good cause is shown, consideration will be given to exterding this response time.
