NOTICE OF VIOLATION l 1

New York Power Authority Docket No. 50-286 Indian Point Unit 3 Nuclear Power Plant License No. DPR-64 As a result.of the inspection conducted on February 28 to April 17, 1989, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violation was identified:

Licensee Technical Specification, Section 6.8.1 requires that written procedures, surveillance and test activities, as referenced in Appendix A of Regulatory Guide (RG) 1.33, November 1972, be established and implemented to ensure that safety-related equipment will perform their intended function.

Licensee's Administrative Procedure AP-4, " Procedure Adherence and Use,"

Section A.1, Revision 8, requires that procedures shall be followed.

Contrary to the above, on March 8, 1989, test technicians failed to implement the instructions of Section 2.8 of the test procedure ENG 259E,

" Service Water Pump No. 35, Functional Test," Revision 1 in that the technician performing the test failed to secure the pump when the vibration level observed on the pump motor exceeded the procedural limit of 3.5 mils.

This is a Severity Level V violation, Supplement I.

Pursuant to the provisions of 10CFR 2.201, the New York Power Authority is hereby required to submit to this office within thirty days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved;-(2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where-good cause is shown, consideration will be given to extending this response time.
