DCS No: 050247861020 Date: October 21, 1986 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-I-86-84 This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. The information is as initially received without verification or evaluation, and is basically all that is known by the Region I staff on this date. i Facility: Licensee Emergency Classification:

Consolidated Edison Company of Notification of Unusual Event

, New York, Inc. Alert Indian Point Unit 2 Site Area Emergency Buchanan, New York General Emergency D/N 50-247 _X_ Not Applicable

Subject:

UNPLANNED SHUTDOWN IN EXCESS OF 48 HOURS At 9:36 a.m. on October 20, 1986, the reactor tripped due to failure of a safety injection logic relay which demands a reactor trip upon a safety injection actuation.

No safety injection signal was present at the time. The licensee plans to repair the relay and associated wiring. During the ensuing transient, difficulties were encountered with two of three auxiliary feedwater (AFW) pumps. The turbine driven AFW pump steam supply line safety valve lifted; and, one of two motor driven pumps tripped on overcurrent following auto-start. The motor driven pump was subsequently restarted from the control room and then operated normally. The third AFW pump started and 4

operated properly and the unit was placed in a stable hot shutdown condition.

The licensee pl ns to remain shut down for 3 to 4 days to continue troubleshooting .

efforts on the protection system relays and auxiliary feedwater system. In addition,

other various outage maintenance activities are planned.

The resident inspector is monitoring the licensee's actions.

The State of New York has been notified.
