Gentlemen:

I Subject: 50-289/87-01

, Enclosed is the report of the NRC team inspection conducted by Mr. R. J.

l Paolino on January 12-21, 1987 at the General Public Utilities Nuclear

corporate office in Parsippany, New Jersey and the Three Mile Island, Unit 1

facility in Middletown, Pennsylvania, of activities authorized by NRC Operating

License No. DPR-50. The team's findings were discussed with you and members

of your staff at the conclusion of the inspection. The inspection reviewed

your implementation of a program for establishing and maintaining the quali-

fication of electric equipment within the scope of 10 CFR 50.49. Within these

areas, the inspection consisted of selected EQ files, procedures and records,

interviews with personnel and observations by the inspectors. The inspection

determined that you have implemented a program to meet the requirements of 10

CFR 50.49 except for certain deficiencies identified in the enclosed inspection

report.

As a result of this inspection, it appears that certain of your activities

have been identified that were not conducted in full compliance with NRC

requirements as set forth in the Notice of violation, enclosed herewith as

Appendix A. This violation has been categorized by severity level in

accordance with the " General Statement of Policy and Procedure for NRC

Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement Policy 1986).

Eight other apparent violations have been identified in your 10 CFR 50.49

program implementation, enclosed herewith as Appendix B, are classified as

Potential Enforcement Items. With regard to these items we are considering

appropriate enforcement action and will inform you of our decision in the

future. The eight Potential Enforcement items include 1) failure to maintain

the EQ files in an auditable form to permit verification of adequacy and

acceptability of qualification documents used in qualifying each item of

electrical equipment, 2) failure to demonstrate that the black Durez

(phenolic) Limitorque limit switch cam shaft was qualified for use in

! containnent prior to this inspection, 3) failure to demonstrate that cable

insulation resistance for Rockbestos Cable had been considered with respect to

l instrument accuracy requirements, 4) failure to demonstrate qualification of

installed Static-0-Ring Pressure Switch versus test specimen, 5) failure to

demonstrate qualification of Kerite Splice and dispositioning of Test -

Anomalies, 6) Inadequate and missing performance data necessary in evaluating

Steam and Radiation Test results and temperature effects on instrument accur-

acy, 7) no traceability or comparison analysis to demonstrate qualification of

Weed RTD's, 8) failure to demonstrate that cable insulation resistance for

Brand-Rex cable had been considered with respect to instrument accuracy

requirements. Your corrective actions regarding the Potential Enforcement

Items should not be delayed pending future NRC inspection or further action by

the NRC regional office.

You are required to respond to this letter. In preparing your response, you

should follow the instructions in the Notice of Violation, Appendix A.

The response directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL-96-511.
