Subject: Canbined Inspection Report No. 50-289/89-15 and 50-320/89-06
A routine, unannounced, safety inspection was conducted at 'Ihree Mile Island
Station Units 1 and 2 by S. Sherbini on July 24-28,1989. 'Ihe purpose of the
inspection was to review the Unit 2 ortyanization and staffing and the status of
the hot particle control program in Unit 2. Also reviewed during this inspection
were the cirtnstances connected with an incident involving a suspected intake
of radioactive material. 'Ihe inspection was a cambined Units 1 and 2 inspection

because of the many shared prvpaus between the two units that inpact on the
inspection area.
Within the scope of the inspection, no violations were identified. However,
'
several areas for' improvement were identified and are detailed in the attached
inspection report. 'Ihe results of the inspection indicate that although a number
of improvements have been made in the hot particle control program, the results
do not appear to have been fully satisfactory, and more effort to implement
further improvements appears to be warranted in this area. 'Ihe review of the
suspected intake incident also indicates that your critique process following
such incidents could be improved. Details are contained in sections 6-8 of the
enclosed report.
