Inspection Report Nos. 50-289/90-15 and 50-320/90-8.
During the period from August 7, 1990 to September 21, 1990, members of the NRC
Region I staff conducted routine safety inspections at the Three Mile Island
Nuclear Station. At the conclusion of the inspection period, Mr. D. Beaulieu,
of my staff, summarized the inspection findings to Mr. Hukill for Unit 1 and
Mr. Kuehn for Unit 2.
Based on the results of this inspection, it appears that two items concerning
procedures were not in full compliance with NRC requirements as set forth in
the enclosed Notice of Violation. A surveillance procedure failed to provide
adequate guidance in establishing the initial conditions for testing of a
Reactor Protection System function which could have resulted in a Reactor
Protection System function not being fully tested. Also, a maintenance
procedure stated that a specific checklist be performed to verify the alignment
of redundant safety equipment when taking a component out of service.
No
checklist existed and none was performed.
In both cases, the weaknesses in the
procedures existed for an extended period without action by your staf f to
address the problem.
