L PPRy m2. 2 SCOPE This instruction covers the processing of Component Modification Cards (CMC's) to the point of Field /Desion Engineering verification. It does not include original design organization approval and/or design review.

2.3 RESPONSIBILITIES 2.3.1 Tield Stractural Engineerino Sucervisor Responsible for the implementation of field design change control reasures associated with various struct-ural supports (e.g., Cable Tray Supports, Conduit Supports,etc.) -

The FSEG Supervisor shall maintain a written list of Field / Design Engineers authorized to approve struct-ural support related CMC's.

2.3.2 Field / Design Engineer Responsible for the origination of CMC's documenting l Design changes / Deviations to various structural l suppor.ts .  ;

Approval authority for CMC forms used for documenting Design change / deviation has been delegated to Field /

Design Engineers as authorized by the FSEG Supervisor.

2.3.4 Technical Services Grouc (TSG)

Responsibilities as outlined in their procedures. TSG to receive infomation and CMC's for Design Verification Action.

3.0 INSTRUCTIONS 3.1 DESIGN CHANGE / DEVIATION (CMC - BLUE CARD) 3.1.1 CMC's shall be issued to the FSEG Clerk by the Document Control Center (DCC).

3.1.2 On an as nee.d basis, the Field / Design (F/D) Enginear will obtain CMC's from the FSEG Clerk.

3.1.3 The FSEG Clerk will maintain a log en CMC's issued

, which will include the following infomation:

a. CMC Serial Number.

'b. Date issued to F/D Engineer.

c. F/D Engineer's Initials h d. Date returned for Distrubution.

leted, it shall be signed by an authorized F/D Engineer and returned to the FSEG Clerk for distribution.

'<P3?p The FSEG clerk will log the date returned and make the following distrubution.

a. DCC - - - Original
b. Technical Services Group - - - 1 Engineering, Office Use Only copy
c. Field Structural Engineering - - - 1 Engineering, Office Use Only 1 Copy I Copy (s) of CMC's shall be transmitted to Technical Services Group using Figure 1 as a cover sheet.

a. When a 3 Part OtC requires revision, a new CiC (Figure 2 of Ref.1-A) with a new number will be issued.

b. Complete steps a through j of attachment 1.
c. The face of the new Cf1C shall contain the statement "This Clic voids and supersedes 3-Part OtC Number .
d. When a 3-Part CMC is required to be voided, a new CMC (Figure 2 of Ref.1-A) with a new number, will be issued.
e. The face of the new C?1C shall contain the statement "This CMC voids

- not supersedes 3 Part CliC Number .

3.3 DESIGN VERIFICATION The field design review and the securing of original ~

design organization approval shall be the responsibility of the Technical Service Group. C?iC's and associated calculations (if provided) shall be transmitted to Technical Services using Figure 1 as a cover sheet.

FOUR DAY DISTRIBUTION a!'(FORg41ATl0h~ '

When requested by the appropria.te craft personnel, the C641 Y DiC may be reproduced and distributed for construction in the following manner:

a. The reproduced C?1C copies shall be :tamoed in accord-PPRV ance with Figure 2 and signed by he originating or cognizant Field / Design Engineer. j j
b. Distribution shall be made to all parties required to perform the work.
c. The originci CMC, upon completion of reproduction, shall be oistributed in accordan~ce with 3.1.5 of this instruction.

E TEXAS UTILTTIES SERVICES INC. INSTRUCTION REVISION PAGE ATTACHMENT 1 CP-EI-4.6-7 1 7-18-80 4 of 7 CMC FORM COMPLETION The Component Modification Card Form (CMC) (Figure 2 of Ref.1-A) is completed as follows.

a. Serial Number - Prenumbered or numbered through detailed application instructions as required.
b. Section 1, Application - State generic category of work.

Weld Mod, Q, Non-Q--Check appropriate block.

Design Change / Deviation--cross out changes -

or deviation, one which does not apply.

Enter "N/A if not a change or deviation to design,

c. Section 2, DWG No. - Enter the complete number and revision of the I affected design basis drawing (s).
d. Section 3, Line No/ Component No. - Enter the specific identification number of the component to be modified; hanger number or N/A as applicable, etc.
e. Section 4, Reason for change - State briefly but concisely the reason for change. If to implement a change previously approved by the original design; clearly state this fact; (e.g. to implement DE/CD 8600).
f. Section 5, Instructions - Describe completely and accurately the change to be made. Enter "N/A" in the

" Remove" & " Add" blocks.

g. Secticn 6 - Provide sketch indicating the existing and/or new l arrangement when necessary for clarificaiton (additional '

pages may be added and must be numbered and identified with the CMC.)

i. Section 8, Approved by - An authorized Field / Design Engineer shall approve the Change / Deviation. (See Section 2.3 of instructions)
j. Section 9, Distribution - See section 3.1.5 of instructions. If additional copies are designated in in Section 9, it is the responsibility-of the originating organization to re-produce and distribute the copies. These

. copies will be marked "For Engineering and Office Use Only."

REVISION TO CMC"5 Revision to CMC cards may be handled by the following two methods:

Method 1. Using Original Card - Copy of original CMC will be made prior to revision and will remain in DCC for his-orical reference. Original card may then be revised as required. In addition the following will be completed.

aa. The appropriate revision numcer shall be placed adjacent to the serial number.

bb. The CMC card shall contain the following state-ment:

"This revision voids and supersedes Document rNFO RMATION Serial number , Revision .

VOIDING CMC'S When an occasion ariseo where a CMC has been issued and is for some reason is not needed, it must be voided.

To void a CMC, the original card must be revised in accordance with Method 1 above with the exception of statement per bb. Instead the CMC shall clearly be marked " Void - not superseded."
