, Changes / Deviations to various structural suoports.

Approval authority for CMC forms used for documenting Design Change / Deviation has been delegated to Field / Design Engineers as authorized by the FSEG Supervisor.

2.3.4 Field Structural Engineering Grouo Clerk Responsible for clerical responsibilities as assioned by the Field Structural Engineering Group.

2.3.5 Technical Services Group (TSG)

Responsibilities as outlined in their nrocedures. TSG to re-ceive information and CMC's for Design Verification Action.

3.0 INSTRUCTIONS 3.1 DESIGN CHANGE / DEVIATION (CMC - BLUE CARD) 3.1.1 CMC's shall be issued to the FSEG Clerk by the Document Control Center (DCC).

3.1.2 On an as need basis, the Field / Design (F/0) Engineer will ob-tain CMC's from the FSEG Clerk.

3.1.3 The FSEG Clerk will maintain a log.on CMC's issued which will include the following information:

Once. the CMC is completed, it shall be signed by an ' authorized F/D Engineer and retur'ned to the FSEG Clerk for distribution.

3.1.5 The FSEG Clerk will log the date returned and make the follow-ing distribution:

Copy (s) of CMC's shall be transmitted to Technical Services Group using Figure 1 as a cover sheet.

3.2 3 PART CMC 3.2.1 3 Part CMC's that have been issued to the Field prior to 2-4-80 may be revised or voided by the following method.

a.

When a 3 Part CMC requires revision, a new CMC (Figure 2 of Ref'.1-A) with a new number will be issued.

b.

Complete steps a.through j.of attachment 1.

The face of the new CMC shall contain the statement "This c.

CMC Voids and Supersedes 3-Part CMC Number d.

When a 3-Part CMC is required to be voided, a new Cf1C (Figure 2 of Ref.1-A) with a new number, will be issued.

organization approval shall be the responsibility of the Tech-nical Service Group. CMC's and associated calculations (if provided) shall be transmitted to Technical Services using i

Figure 1 as a cover sheet.

3.4 FOUR DAY DISTRIBUTION 3.4.1 When requested by the appropriate craft personnel, the CMC may be reproduced and distributed for construction in the following manner:

a.

The reproduced CMC copies shall be stamped in accordance with Figure 2 and signed by the originating or cognizant Field / Design Engineer.

b.

Distribution shall be made to all parties required to per-form the work.

5 of 8' CMC FORM COMPLETION The Component Modification Card Fom (CMC) (Figure 2 of Ref.1-A) is completed as follows.

a.

Serial Number - Prenumbered or numbered through detailed application instructions as required.

b.

Section 1, Application - State generic category of work.

Weld Mod, Q, Non-Q--Check appropriate block.

Design Change / Deviation--cross out changes or deviation, one which does not apply.

Enter "N/A if not a change or deviation to

design, c.

Section 2, DWG No. - Enter the complete number and revision of the affected design basis drawing (s).

d.

Section 3, Line No/ Component No. - Enter the specific identification number of.the component to be modified; hanger number or N/A as applicable, etc.

e.

Section 4, Reason for change - State briefly but concisely the reason for change. If to implement a change previously approved by the original design; clearly state this fact; (e.g.toimplementDE/CD8600).

f.

Section 5, Instructions - Describe completely and accurately the change to be made. Enter "N/A" in the

" Remove" & " Add" blocks.

g.

Section 6 - Provide sketch indicating the existing and/or new arrangement when neces3ary for clarificaiton (additional pages may be added and must be numbered and identified with the CMC.)

Section 8, Approved by - An authorized Field / Design Engineer shall approve the Change / Deviation.

(See Section 2.3 of instructions)

j. Section 9, Distribution - See section 3.1.5 of instructions.

If additional copies are designated in in Section 9, it is the responsibility-of the originating organization to re-produce and distribute the copies. These copies will be marked "For Engineering and Office Use Only."

REVISION TO CMC"S Revision to CMC cards may be handled by the following two methods:

Method 1.

Using Original Card - Copy of original CMC will be made prior to revision and will remain in DCC for his-orical reference. Original card may then be revised as required.

In addition the follo' wing will be completed.

aa. The appropriate revision number shall be placed adjacent to the serial number, bb. The CMC card shall contain the following state-ment:

"This revision voids and supersedes Document MORMiGON Serial n-ber

VOIDING CMC'S When an occasion arised where a CMC has been issued and is for some reason is not needed, it must be voided.

To void a CMC, the original card must be revised in accordance with Method 1 above with the exception of statement p.er bb.

Instead the CMC shall clearly be marked " Void - not superseded."
