Gentlemen:

SUBJECT: REPORT NOS. 50-338/86-04 AND 50-339/86-04

On February 3 - March 2, 1986, NRC inspected activities authorized by NRC License

Nos. NPF-4 and NPF-7 for your North Anna facility. At the conclusion of the

inspection, the findings were discussed with those members of your staff

identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report. Within these

areas, the inspection consisted of selective examinations of procedures and

representative records, interviews with personnel, and observation of activities

in progress. Based on the inspection of the reactor protection system and

discussions held between your North Anna facility management staff and our

inspectors, described in paragraph 12 of the enclosed inspection report, we

request confirmation of your corrective action to further preclude the conditions

described in IE Information Notice No. 85-98.

The inspection findings indicate that certain activities violated NRC require-

ments. The violations, references to pertinent requirements, and elements to be

included in your response are presented in the enclosed Notice of Violation.
