SUBJECT:' -SAFETY SYSTEMS OUTAGE MODIFICATIONS AND MAINTENANCE TEAM INSPECTIONS-

AT NORTH ANNA NUCLEAR POWER STATION

(INSPECTIONREPORT 50-338/89-200 and 50-339/89-201)

This letter forwards' the report and executive sumary of the installation and

testiphase of the safety systems outage modifications inspection (SSOMI) and:

maintenance team inspection'(MTI) conducted by the NRC's Office of Nuclear

Reactor Regulation during the weeks of May 8 and May 22, 1969. At the .

-conclusion of the inspection, the inspection team discussed the findings with

you and members of your staff identified in Appendix B of the enclosed inspec-

tion report.~

The purpose of the installation and test phase of the 550MI was (1) to

determine, through an examination of specific design changt pac?tges and

- . engineering. work requests, that installation of the modifications conformed

with the design and installation requirements and (2) to verify that the

modified systems and components have been adequately tested to ensure that

they are capable of perforning their intended functions. The MTI was

performed to determine (1) whether plant components have been properly

' maintained so that they are available to perform their intended function

and (2) whether the maintenance process provides for prompt repair of plant

components in relationship to their safety importance.

The team generally found your modification and maintenance programs to be

effectively implemented. The inspection team identified weaknesses related

to performance of vendor surveillance activities, lack of detail in some

procedures. some administrative control problems, an excessive number of

field changes issued against the relatively older modification packages.

u ntimely corrective-actions in response to identified deficiencies, an

overly narrow scope of manual valve maintenance reviews, an inadequate

' root cause' evaluation progr:m, and incorrect pressurtrer spray valve

position. indication. Hc ,er, the team found that site personnel were

knowledgeable about their work and that supervisors were monitoring field

activities closely.

The current site staffing levels appear to be strained in several areas of the

organization. This situation has delayed the iniplementation of several important

site initiatives. We are concerned that planned programs will suffer if the

available personnel resources are not adjusted to a level commensurate with the

site workload.

Although no formal response to this report is required, some of the findings

identified by the inspection team may be potential enforcement items. Any

enforcement actions will be initiated by Region II under separate

correspondence.

In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosure

will be placed in the NRC Public Document Room.
