AD-115' aav 0 {. -. t 1.0 INTROD CTION \ Temporary modificati'ons to the normal configuration of equipment and electrical circuits are required during the Startup Program
-to facilitate initial operation and testing of equipment and
, , systems and for certain corrective maintenance items (such as removal of'a valve and installing blank flanges).

These temporary modificatiiins may include, but are not limited to: lifted electrical wires; installation of electrical ju=pers (temporary wiring); and installation of temporary mechanical devices, such as piping, flushing connections, pipe' caps, and blind flanges.. Also included is equipment temporarily modified such as removing valve internals or disconnecting tubing. These temporary modifications must be controlled, identified, and docuhanted to ensure subsequent restoration to normal. 2.0 PURPOSE . - This instruction establishes the documents and procedures employed
- to identify and control equipment.and elcetrical circuits celi-
- berately and temporarily modified. '
3.0 SCOPE 3 . '1 Prercquisite Test Program This instruction applies to temporary modifications initiated during the prerequisite test program. . 3.2 Limitations 3.2.1 This instruction does not apply to equipment removed or disassembled for inspection, calibration, repair, or maintenance which is being performed and documented in accordance with other established startup procedures or instructions. - 3.2.2 This instruction does not apply to electrical test equipment test 1 cads, which includes all cicetrical
- test 1 cads between test equipment and system circuits used to perform component testing, circuit troubic-shooting or to record data. -

- with instructions in Exhibit D and place the-log sheet in the log book under the appropriate sub-system. The Startup Engineer shall ensure the log index is filled out. The cogni: ant Startup Engineer shall concur with the temporary modification and sign the log sheet authori:ing the modification.
The tags shall be attached when the temporary modification is accomplished. S.3 Restoration of Temporary Modifications 5.3.1 Restoration of temporary modifications shall be accomplished by the cogni: ant Startup Engineer. The discipline EGS may authori:e restoration of temporary modifications in the Startup Engineer's absence. The log sheet shall be signed and dated in accordance with the instructions in Exhibit D. All tags associated with the modification shall . (' be removed and destroyed. The log sheet shall >U ,bc placed in the inactive file. . 5.3.2 Temporary !!odifications that have not been restored upon System Release for Preoperational Testing. shall beccme the responsibility of APS Startup. - Part of this release process shall include the review of the Temporary Modification Log.
- 5.4 Review 5.4.1 Temporary Modification log.index shall be

reviewed monthly. 5.4.2 The Review should consist of examination of
. documents and the systems to determinc if the temporary modification is still required and if adequate steps have been taken to restore or document the modification. All discrepancics
- shall be corrected immediately upon discovery.
This includes ti.e replacement of missing tags that have inadvertently been removed from system components. Documentation of this review shall - be made by the reviewer signing and dating in the spaec provided on the Log Inder verifying that th. l.og reficcts the current status of temporary b mod fications within the designated system. w
