Dear Mr. Keppler:

Reference (a) transmitted the results of an inspection conducted by Messrs. N. J. Chrissotimos and S. G. Dupont on July 21 through August 29, 1980 of activities at Quad Cities Station.

Appendix A to Reference (a) identified two apparent items of noncompliance requiring additional response.

hRC Dock 2t No. 50-254 COMMONWEALTH EDISON COMPANY ATTACHMENT A NOTICE OF V OLATION The items of non-coepilance identified in Appendix A to the NRC letter dated October 15, 1980, are responded to in the following paragraphs.

1. Unit 1 Technical Specifications, Section 6.2.A.I. requires that opera-tion of the reactor and other systems and components involving nuclear safety of the facility be performed in accordance with detailed written procedures and applicable checkoff lists.

Contrary to the above, on August 17, 1980, while performing surveli-lance Q15 130-2 "RCit Pump Operability Test" :he operator failed to return the RCIC to its normal operating conf sguration as stated in Step 8.

2. Unit i Technical Specifications, Sect.on 6.2.A.I. requires that opera-tion cf the reactor and other systems and components involving nuclear safety of the facility be performed in accordance with detailed written procedures and applicable checkoff lists.

Contrary to the above, during the period of August 17, to August 18, 1980, the RCIC flow controller was not in its normal operating con-figuration as stated in Step C 5 of Procedure QOS 005-2 " Normal Control Room Inspection and Shif t Turnover Panel Checks."

Corrective Actions Taken and Results Achieved:

Upon identification of this event, the RCIC controller was returned to its normal operating configuration. An on-site investigation was conducted by the Station to review the situation with the Individuals involved. A dis-cussion was held with all of the NS0s to re-emphasize the need to adhere to Station procedures. The Shif t Technical Advisors are accompanying the NS0s during their shif t turnover panel checks until improvement is demonstrated.

Corrective Actions To Be Taken To Avoid Further Non-Como11ance:

A revision to Station procedure QOS 005-51, " Operations Department Weekly Summary of Daily Surveillance Reading Sheet" is in progress which will require the Unit Operator to check the status of the RCIC controller once per shift to verify that it is in the AUTO mode. This revision should be laplemented by December 1,1980.
