SUBJECT: QUAD CITIES INSPECTION REPORT 50-254/99009(DRP);

50-265/99009(DRP)

Dear Mr. Kingsley:

This refers to the inspection conducted April 21 through May 31,1999, at the Quad Cities i

Nuclear Power Station. The purpose of the inspection was to determine whether actitities

authorized by the license were conducted safely and in accordance with NRC requirements.  ;

The NRC conducted routine resident inspections in this period. Based on the inspectors'

findings, a safety-conscious approach characterized the overall conduct of activities at

Quad Cities. At the conclusion of the inspection, the findings were discussed with members of

your staff.

During this inspection period, a Unit i reactor trip occurred. The trip was caused by interactions I

of the reactor water cleanup system relief valves with the scram discharge volume. Previous

problems with the relief valves lifting were not adequately addressed. Separately, some i

operator performance problems were identified including a failure to properly position a high  ;

pressure coolant injection valve, a surveillance test performed on the wrong unit, and a failure

to recognize core power computer problems in a timely manner.  !

Based on the results of this inspection, the NRC has determined that one violation of NRC

requirements occurred. Operators failed to properly position the Unit 2 high pressure coolant

injection discharge valve during performance of a surveillance test. This violation is being

treated as a Non-Cited Violation (NCV), consistent with Appendix C of the Enforcement Policy.

T; 1s NCV is described in the subject inspection report. If you contest the violation or severity

level of this NCV, you should provide a response within 30 days of the date of this inspection

report, with the basis for your denial, to the Nuclear Regulatory Commission, ATTN: Document

Control Desk, Washington, DC 20555-0001, with copies to the Regional Administrator,

Region til and the Director, Office of Enforcement, United States Nuclear Regulatory

Commission, Washington, DC 20555-0001.
