L c L On April 19,1990 members of the Nuclear Reactor Regulation's (NRR) and Commonwealth Edison Company's Staffs conducted a technical meeting to discuss the cracks identified in the quad Cities Unit 2 Reactor Vessel Head. During that l meeting, Commonwealth Edison indicated that we believed the cracks were related >

to the fabrication processes of the vessel head. Theiefore, Commonwealth Edison committed to research the fabrication history ofits Boiling Water Reactors (BWR).

The referenced letter provided the schedule for submitting the summary of the fabrication history for our six (6) BWR vessels.

. JAttachment 1 provides the fabrication history summary for the Quad .

. Cities Unit 1 Reactor VesselEThis summary was developed through the review of a - 1 limited population of records, however, provides the necessary information required ,

. to identify target areas for inspection. Attachment 2 provides a listing of Contract Variations which were identified through the review of General Electric Quality

Control Checklists' The review of the Contract Variations has not yet been completed and will be provided to your Staff by December 31,.1990.

The Quad Cities Unit 1 Reactor Vessel was fabricated entirely by Babcock

. & Wilcox. The summary.ind!:ates that fabrication mismatches, which are  ;

considered to'be significant for develo pment of an inspection plan, were identified at l

. the head-to-flange, flange-to-shell and. uppermost shell-to-shell welds. The review of.

k . limited fabrication records indicated that back cladding' of these welds was 7erformed by subarc welding; however, a more detailed review of the specific weld f abrication records has indicated ti t the back cladding operation was performed by a manual shielded metal are welding process.

i The fabrication history of the Quad Cities - Unit 1 reactor i vessel, as detailed below, is based on the review of the six 1 microfilm spools from B&W and the G.E. " Quality History" 4

'l The-vessel flanges were made # rom SA336 Code Case 1332-1 I forgings (i.e, a nickel-chrome-moly-low alloy steel) with a- .)

vanadium content of .02 .04 percent shown on the Certified Material Test Reports (CMTR's). The torus sections, dollar i plates,'and shell were fabricated from SA302 Grade B Code.

Case 1339 plates. CMTR's for this manganese-nickel-moly low i '

alloy steel did not report vanadium. Except for a plate in the top head' torus which did not show a grain size and one .

plate in the upper intermediate shell with a grain size of four., all plates _had a grain size of five or finer. Plate thickness for-the top head was 4" and for the bottom head .

dollar plate and lower torus 8".- The CMTR's for the shell  !

plates and' bottom head-dollar plate show 6-1/4"_ thickness but the ASME N-1A Code Data reports lists them as=6-1/8". .,

Submerged arc welding material is a special wire and a Linde p #80 flux producing a weld deposit which matches-the plate

material. Shielded metal arc welds use E8015-C3 electrodes, .!

L a nickel-coly. alloy. Most heats of E8015-C3 had a vanadium-  !

i- content of .01 percent; however, a heat with the-highest '

F vanadium, .028 percent, was used on the. repair and weld

build-up of the top head assembly to flange weld. . :j
2. General ~ Fabrication Sequence Fabrication was-performed by B&W, primarily in their j Mt. .Vernon, Indiana-shop; however, the assembly:of the lower  ;

vessell(i.e., the bottom shell ring, upper and-lower: torus 3 andidollar plate) was field welded together on a barge in Madison, Indiana reportedly because of a labor-dispute. The

- ' fabrication' problems detsiled below are summarized by location and illustr:ted c? the sketch in Attachment G.  ;

A. Too Head-Assembly i

This asserbly consists of a dollar plate, torus section, and flange, The. torus section is composed of six roll formed : plat es submerged arc welded (SAW) together. The torus to dollar plate weld had a miumatch up to 5/8" and

. weld gap at the root up to 1-3/4" (the drawing specifies 3/4" minimum but.no maximum). Nevertheless, the weld was made with the fit-up condition by SAW. Preheat was '

-maintained at 300*F for eight hours but then dropped to room temperature prio to postweld heat treatment (PWHT).

This reject condition was cleared by ultrasonically testing (UT) the weld. After a PWHT which involved an

.v CMICASO E' , excessive heating rate (temperature went from 600*F: up to- .;

1035'F in thirty-five minutes), UT of the circle seam was

f. required. This revealed a deep' defect which was weld:-

repaired-(see Attachment A), UT cxamined and then PWHT.-

Two locations.on the flange I.D. with a cumulative. length i of about 1/3-the circumference had undersized areas - up '

to 5/16" deep - which were weld built-up and PWHT hy B&W. .

During fit-up of the top head assembly to flange weld Y excessive mismatch - up to.3/4" - and weld gap at the- 1 i

root-(the O.D. for this weld) of 1-1/2" was encountered.

The weld andLweld l' alld-up on the I.D. was made using.

SAW. - Upon : removal of the O.D. backing strip a crack was detected visually in the weld. This repair and the O.D., A weld build-up were made using shielded' metal' arc welding (SMAW). The I.D. and O.D. weld build-up was blend ground to a 3:1 taper in.the transition area.

B. Shell~& Flance

.The shell consists of four rings designatea as the top, 1 upper intermediate, lower intermediate and bottom. Each'  ;

ring'normally is composed of three-plates electroslag

~'

welded together and then the ring 'is given a- quench :and temper-heat treatment. However, each ring'except,the upper intermediate shell had a dimensional discrepancy which Babct7k & Wilcox (B&W) resolved by cutting a fourth 3 seam'in-the rings, expanding the diameter and making.a.  ;

fourth electroslag weld. Each ring also had base metal weld repairs ~to one or more plates. The shell rings are n

welded together (circumferentially).by SAW with the lower i E intermediate to bottom-shell; weld being the finalfclosure' weld for the vessel. The shell flange was received from.

the forging supplier with numerous defect cavities and

. low. areas on the sealing face which required' major weld repair. The . 300*F minimum preheat was- not maintained to- t PWHT. Instead the repairs were heated to -400-800*F< for a minimum.of two hours'and then allowed to cool to room ,

temperature. For the flange to top shell weld, a -

mismatch of up to 1-1/32" occurred at the I.D. (up to 3/8" on O.D. but this is-allowable). 'The weld was made . -

by SAW, weld: built-up by SMAW on the I.D. and then-ground to a 3:1 taper. The joint between the. top shell (piece- i Mark 60) and the upper intermediate-(Mk. 59) had,a mismatch of up to-1-1/2" as shown'on Attachment B. The excess material was air arc gouged away and the weld made 4

'using SAW with additional weld deposit serving as weld ~

build-up so that the transition was blend ground to a 5:1 taper. For the joint'between the upper to lower ,

intermediate shell, there were two areas each on the I.D.

and O.D. which were 7/16" mismatched; otherwise the joint met the 3/8" allowable mismatch. The weld was made by SAW in this condition. 'In the final closure weld, the bottom shell (Mk. 57) witn the bottom head assembly welded to it, is joined to the lower intermediate shell 4v

attached. As shown on Attachment C, this joint had about =

7/8"Jaismatch on the I.D. and'O.D. with a large weld gap i (almost 3"). The; weld and weld build-up was_made by SAW-

. .i and the; transition blend ground to a_3:1 taper. However, when the weld was two-thirds complete the preheat was '

dropped'during removal of the backing ring and strongbacks. Magnetic particle testing (MT) and UT of i we the. weld showed no indications.- In' addition,~after the i final'PWHT this weld receives MT,-UT and RT  ;!

(radiography). Based on this it was accepted. The i bottom shell had an O.D. weld repair-about 4" deep due to

.d removal of a defect found by RT adjacent to the Mk. 7_ l recirculation inle' nozzle.

C. ' Bottom Head Assemb.ly The bottom head history has been obtained from the G.E.

- checklist ~and the contract Variations. The six miclofilm spools did not contain bottom head fabrication records i i _.for Unit 1. The bottom head assembly is' comprised of an H

" upper torus, lower torus, and dollar plate. -The upper .I torus is made of six formed plates-welded by SAW. The j lower torus consists of four plates welded together.by 1 SAW. The' dollar plate had two weld repairs on the O.D. _ t where boat samples 5-1/8" long X 1-3/4" wide X 1-3/8" '4 deep.were taken for test purposes. -The dollar plate to lower torus joint was shown to have been weld built-up by -

SAW to correct for a mismatch, welded'by SAW^and then had an;SMAW repair to the weld < joint due to indications found  !

by UT (reference to1 Contract Variations CV #122-11 and --

21 not found in the records). A contradiction is shown.

in G.E.'s-summary list of CV's where CV #122-21 is' stated

to.lua for:a' mismatch between the upper-to lower torus.

The joint between the' bottom head assembly and lower

~

shell had a mismatch up to 1-5/8" which required metal:

removal and SMAW weld build-up as shown on Attachment D. ,

The weld was made also.using SMAW and the transition blend grount ;o a taper between 3:1 and 6:1.

3. Cladding The' sealing surfaces of the flanges and the.I.D. of the-i dollar plate / lower torus assembly are SAW cla~d with Inconel. .

The-only problem identified.with Inconel cladding was two 1

. small exposed base metal areas in the final! bottom head assembly near the dollar plate weld, 3-1/2".away from the

~

shroud leg. These were found after the field hydrostatic  ;

test'and SMAW repair welded without PWHT using a half-bead,

, , . temper bead technique and invoking Code Case 1401. After completion of the Inconel Pad Build-ups located at the top of the upper-torus in the bottom head B-1 subassembly, the preheat was dropped. A UT examination showed no indications 4

- and-the. variation was' accepted. All other vesselOID surfaces,' including-the flanges, were clad with a 1/8"-

minimum thickness of stainless steel. Contrary.to<the G.E.

checklist, _the records indicate-that through a deviation the-

, top head dollar plate and torus were clad as an. assembly using the SAW-6 wirs method. This automatic process uses 338L wires with welding parameters of 400 amps, 37 volts-and 8" ipa travel speed. All SAW cladding was sampled-(single electrode procedure WR8 and WR59 show the first bead and'then every fifth bead) for chemical analysis and ferrite determination. Ferrite was required to be 5-15 percent as det ermined by a' Magne Gage. None of the test results were found in the records. The top head dollar plate / torus assembly had numerous clad repairs with some extending up to 1/2" into base metal (see Attachment E). The top center 9" diameter of this assembly is not clad automatically but by SMAW. . The flange I.D. was clad by the SAW - 6 wire method and~the chemical analysis sample from bead #7 was below the specification requirements. An " accept" ticket-stated that the bead wan removed, reclad and a new sample rechipped with acceptable chemistry per Lab #6391 and 6467. he flange.to top head assembly was back clad be " 7' - rode SAW. The wire and welding parameters for the single actrode SAW cladding are the same as the 6 wire method. The flange I.D.

, o and circle seam back clad area had minor repairs made by SMAW using E308 electrodes. All the shell rings were individually clad by the SAW - 6 wire method. After cladding the upper intermediate shell, the preheat was dropped prior tx) PWHT.

This contract variation (CV #122-25 but not found in records) was accepted by G.E. Minor repairs to the top shell cladding were made at locations shown on Attachment F. The shell-flange I.D.-and, contrary to the G.E. checklist, the flange-to-top shell, weld were also clad by the SAW - 6 wireimethod.

Back cladding of the top shell to upper intermediate shell circle' seam weld was-by SAW (WR'15 or WR 15 Alt. 1 but not found in the' records). These procedures were also scheduled' for back cladding the upper to lower intermediate shell' weld but the records indicate that it was'back clad by SMAW using 5/32" diameter E309 at 140 amps for the initial. layer and then 5/32" and 3/16" diameter E308 at 145/175 amps respectively. Back cladding by SMAW was also used for the closure weld and the bottom head assembly to lower shell weld. The bottom head upper torus was clad with stainless steel by single electrode SAW. Back cladding of the upper to

! lower torus weld is shown as WR 39 which is an Inconel SAW cladding procedure. On Unit 2 the drawing showed WR 39 for back cladding this same weld but actually stainless steel cladding using SMAW was performed. All final clad surfaces are examined by UT to assure proper bonding and PT (dye penetrant) for surface defects. Grinding of the cladding preparatory to these excminations is permitted as required.

Blend grinding of back clad and repair areas was specified.
