2. Reason for the Violation TVA's metallurgical cnalysis of the piping revealed that the piping exhibited a longitudinal abrasion or die mark with numerous carbon steel particles embedded in the piping along the mark.

There was no direct evidence of the origin of the carbon steel particles but TVA believes the contamination most probably occurred during the drawing process. The vendor (Capitol Steel) was contacted with the results of TVA's investigations and has indicated that they agree with TVA's assessment.

Based on the infrequency of this type of defect, both TVA and the vendor agree that thic is an isolated occurrence.

4. Corrective Action Taken to Avoid Furth3r Violations Based on the isolated nature of this deficiency, no actions by the vendor are necessary to prevent recurrence.
5. Date When Full Compliance Will Be Achieved TVA is now in full compliance.
