RESPONSE TO SEVERITY LEVEL V VIOLATION 50-438/84-26 AND 50-439/84-26 INSTRUMENTATION CRAFT FOREMN/ DUAL RATED NOI TRAINED AND QUALIFIED TO APPROPRIATE TRAINING MODULE BEFORE ASSICNENT TO ACTIVITY Description of Deficiency 10 TR 50, Appendix B, Criterion V, and the accepted quality assurance program (TVA-TR-75-1 A, revision 8), Section 17.1. A.5, requires in part that activities affecting quality shall be prescribed by docmented procedures of a type appropriate to the circumstances and shall be accomplished in accordance with these p. ocedures. Paragraph 6.1.6 of Bellefonte Nuclear Plant (BLN) quality control procedure (QCP) 10.30, revision 5, "Craf t Quality Assurance Training," states in part that craf t foremen / dual rates are to be trai,ned and qualified to appropriate construction training modules before being assigned to a new work activity.

Contrary to the above, as of December 11, 1984, activities affecting quality were rnt being accomplished in accordance with documented procedures in that an instrtanentation craf t foreman / dual rate was not trained and qualified to appropriate construction training modules before being assigned to a new work activity.

TVA Response i

1. Admission or Denial of the Alleged Violation TVA admits the violation occurred as stated.
2. Reason for Violation BLN QCP-10.30 requires that craf t foremen cr dual-rated foremen expected to serve as an Murly foreman for a period of five days or more be trained and qualified in the appropriate construction training modules prior to being assigned to a new work activity. The instrtanentation superintendent failed to follow this procedure in that he assigned a dual-rated craf t foreman to supervise a crew on second shift without training and qualifying the employee in the required training modules.

The dual-rated foreman supervised the crew seven days, violating the five-day time limit outlined in this . procedure.

3. Corrective Steps Taken and Results Achieved The dual-rated foreman is no longer supervising a crew and will not supervise another crew until the proper training is received. The violation has been discussed with the instrinnentation superintendent, and he is aware that he violated the QCP. He has been reprimanded for the incident and reminded of his msponsibilities to BLN's quality assurance program.
