Walker's letter dated February 27, -1985 report nunbers 50-438/85-04, 50-439/85-04 concerning activities at the (Bellefonte Nuclear Plant which appeared to have been in violation of NRC resulations . Enclosed is our response to the citation.

BELLEFCNTE NUCLEAR PLANT UNIT 1 RESPCNSE TO SEVERITY LEVEL IV VICLATICH 50-438/85-04-01 CABLE TERMINATED ON TIE WRONG TERMINAL BOARD Deseription of Deficiency 10 TR 50.54(a)(1) requires the Tennessee Valley Authority (TV A) to implenent the Quality Assurance Program described or referenced in its Safety Analysis Repor t. Section 17.1.5 of the TVA QA Program requires that doctznented instructions b3 implenented for all activities affecting quality. BNP-QCP 3.4, revision 4, paragrarh 6.4.2, requires the inspector to certify that the installation complies with the termination portions of-this procedure by signing and dating the termistion slip. The termination slip for cable INC-ECA2-97-8 was signed on January 20, 1982.

Contrary to the above, procedures were nat adequately implenented in that the cable was terminated on terminal board AM and not the required terminal board g AL.

TVA Response

1. Admission or Denial of the Alleged Violation TV A admits the violation occurred as stated.

2.~ Reason for Violation A lack of identification of the terminal boards by the manufacturer resulted in the incorrect termination to the terminal board AM in the panel by the Bellefonte Nuclear Plant (BLN) craf t personnel.

Subsequently, BLN Electrical Quality Control (EQC) personnel failed to detect the incorrect termination because of the lack of identification.

3 Corrective Steps Taken and Results Achieved Nonconformance report (NCR) 3897 was prepared to document the incorrect

~ termination. This deficiency has - been reworked per BLN quality control .

procedure (QCP) 10.6, " Work Release," and NCR 3897 has been closed.

4. Corrective Steps Taken to Avold Fbrther Noneorroliance TVA believes that this is an isolated case based on results of audits previously perfomed weekly by the EQC internal audit pecgram and past perfomance of the functional test program. No significant problens have been identified maarding cables being terminated on the wrong terminal board. In the early stages of constructs on there was a problen with identification of terminal blocks in these particular pieces of equip-ment. An intensive effort to correct this problen was conducted by the

BLN Electrical Engineering Unit (EEU) which con;icted of- providing labels ,.

which clearly identify the terminal bo:rds. The incorrect cable * **
