This letter and attached inspection report presents the findings of a special | |||
inspection conducted by site inspectors of this office on June 16 to June 30, | |||
; 1986 of activities authorized by NRC License No. DPR-73. The findings were | |||
i discussed with fir. T. Demmitt of your staff at the conclusion of the | |||
inspection on July 2, 1986. | |||
Areas examined during this inspection were the conduct of the defueling | |||
- | |||
operations and radiological controls as a result of handling small pieces of | |||
debris from the reactor core bed and in the handling of tools or other items | |||
when being removed from the reactor vessel. Within these areas, the | |||
inspection consisted of a selective examination of procedures and records, | |||
, interviews with personnel, and observations by the inspector in connection | |||
with the circumstances surrounding handling of small pieces of debris and | |||
tools removed from the reactor. | |||
It appears that two of your activities were not conducted in full compliance | |||
with NRC requirements, as set forth in the flotice of Violation, enclosed | |||
herewith as Appendix A. These violations have been categorized by Severity | |||
Level in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C) | |||
published in the Federal Register (49 FR 8583), dated March 8,1984. You are | |||
required to respond to this letter and in preparing your response, you should | |||
follow the instructions in Appendix A. | |||

During the review of the events on June 21, 1986, we identified several errors | |||
which indicate a management breakdown in control of your defueling activities. | |||
, Specifically, a supervisor directed persons working in the reactor building to | |||
take actions without first confirming conditions in the building; supervisors | |||
and workers had not read applicable Radiation Work Pemits and were not aware | |||
of all requirements; there was evidence of inadequate communications between | |||
supervisors; and some supervisors were not attending pre-shift briefings. In | |||
