. NOTICE OF VIOLATION GPU Nuclear Corporation Docket No. 50-320 Three Mile Island Unit 2 License No. DPR-73 As a result of the inspection conducted on June 16 - 30, 1986, and in accordance with the revised NRC Enforcement Policy (10 CFR 2, Appendix C),

published in the Federal Register on March 8, 1984 (40 FR 8583), the following violations were identified:

A. 10 CFR 20.201 requires, in part, that the licensee make, or cause to be made, measurements of radiation levels associated with radioactive materials as needed to evaluate the extent of the radiation hazards that may be present due to the use of radioactive material.

Contrary to the above, on June 15, 1986, the licensee failed to perform a survey sufficient to evaluate the radiation hazard associated with a piece of reactor core debris on the Shielded Work Platform before placing the debris back into the reactor vessel.

This is a Severity Level IV violation (Supplement IV).

B. Technical Specifications, Appendix A, Section 6.11 requires that personnel radiation protection shall be consistent with the requirements of 10 CFR Part 20 and the Radiation Protection Plan.

The Radiation Protection Plan, 1000-PLN-4010.01, Revision 1, dated May 1, 1986, requires that, in part, workers obey posted, oral, and written radiological controls instructions and procedures, including instructions on Radiation Work Permits.

Radiation Work Permit Number 14372 instructions required, in part, that all tools and equipment removed from the reactor vessel shall be scrubbed, wiped, and surveyed prior to relocation to the repair or storage area.

Contrary to the above, on June 21, 1986, multiple examples occurred when separate teams working under Radiation Work Permit Number 14372 removed canister handitng tools from the reactor vessel and placed them in storage without scrubbing and surveying the tools as required.

This is a Severity Level IV violation (Supplement IV).

Pursuant to the provisions of 10 CFR 2.201, GPU Nuclear Corporation is hereby required to submit to this office within 30 days of the date of this Notice, a written statement or explanation in reply including: (1) the corrective steps which have been taken and the.results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved for the violations. Where good cause is shown, consideration will be given to extending your response time.
