                    This letter and attached inspection report presents the findings of a special
                     inspection conducted by site inspectors of this office on June 16 to June 30,
          ;          1986 of activities authorized by NRC License No. DPR-73. The findings were
          i         discussed with fir. T. Demmitt of your staff at the conclusion of the
                     inspection on July 2, 1986.
                    Areas examined during this inspection were the conduct of the defueling
     -
                    operations and radiological controls as a result of handling small pieces of
                    debris from the reactor core bed and in the handling of tools or other items
                    when being removed from the reactor vessel. Within these areas, the
                    inspection consisted of a selective examination of procedures and records,
   ,                interviews with personnel, and observations by the inspector in connection
                    with the circumstances surrounding handling of small pieces of debris and
                    tools removed from the reactor.
                    It appears that two of your activities were not conducted in full compliance
                    with NRC requirements, as set forth in the flotice of Violation, enclosed
                    herewith as Appendix A. These violations have been categorized by Severity
                    Level in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C)
                    published in the Federal Register (49 FR 8583), dated March 8,1984. You are
                    required to respond to this letter and in preparing your response, you should
                    follow the instructions in Appendix A.

                    During the review of the events on June 21, 1986, we identified several errors
                    which indicate a management breakdown in control of your defueling activities.

, Specifically, a supervisor directed persons working in the reactor building to

                    take actions without first confirming conditions in the building; supervisors
                    and workers had not read applicable Radiation Work Pemits and were not aware
                    of all requirements; there was evidence of inadequate communications between
                    supervisors; and some supervisors were not attending pre-shift briefings. In

                 The response directed by this letter and the accompanying Notice is not
                 subject to the clearance procedures of the Office of Management and Budget as
                 required by the Paperwork Reduction Act of 1980, PL 96-511.
