4.0 GENERAL

j- 4.1 Responsibilities t

4.1.1 QA/QC Discipline Engineers

.The QA/QC Discipline Engineers prepare the quality instruction delineating reinspection requirements and attributes.

4.1.? QA/QC Inspectors The QA/QC Inspectors perform reinspection in accordance with this inspection instruction and established project procedures. Reference 3.2 addresses the method to perform reinspection and to document and process the results.

4.2 Policy Activities performed.under this instruction shall conform to the~ policies contained in the latest Comanche Peak Response

Verify that the Restraint or the Support Structure is permanently marked with the proper identification number as I

indicated below. Except for U-Bar restraints, if the permanent marking is not visible due to paint, mark the. j attribute "N/A" on the checklist and note " inaccessible" .n the remarks column.

U-Bar Restraint is identified by the Restraint no. on the metal tag permanently attached to the U-Bar.

Bumper Restraint is. identified by the assembly identification no. shown on the vendor dwg. in Unit 1 and by the Restraint number in Unit 2.

Hard Restraint is identified by the structural detail ,'

no. on Gibbs and Hill structural drawings.

for Honeycomb Restraint, the Honeycomb is identified by g the mark no. on Gibbs & Hill structural drawing and the Structure is identified by the vendor drawing piece numbers.

Moment Restraints are identified by the Restraint no.

on the forging.

Restraints supplied by Westinghouse are identified by the equipment number shown on Westinghouse drawing or by piece number on the vendor drawings.

Support Structure is identiiled by the vendor drawing piece numbers. Identification is necessary of as many pieces as makes the Inspector comfortable that he/she is at the correct support structure.

5.2 Location (2)

Verify, by measurement, the location of the Restraint / Support structure as indicated below.

Location of a Support Structure is verified if, with respect to the Structure, the. location of all the corresponding Restraints on the Structure is per the drawing. The elevation / location of the Support Structure itself is for reference only. If no Restraint is installed, the Inspector shall "N/A" this attribute on the Checklist. If Restraint location is not shown on the drawing, the Engineer shall either "N/A" this attribute or provide Supplemental Instructions for this verification.

Unless shown otherwise on the drawing, the following tolerances apply for location of the. Restraints on the Support Structure:

U-Bar Restraint: 1 2" Bumper Restraint:  ! 1/8" Moment Restraint: t 1/2"

Honeycomb Restraint: 1/4" Hard Restraint: 2 1/4" Location of all Restraints, except Moment Restraints, is verified if, the Restraint location with respect to the pipe elbow is per the drawing. Moment Restraint location needs no verification. Note that location for a U Bar Restraint may be specified as dimension '3' on the HFT (Hot Functional Testing) Card included in the package.

Unless shown otherwise on the drawing, the following tolerances apply:

U-Bar Restraint: 1 1/8" Bumper Restraint: 1 1/8" Hard Restraint: 1/4" Honeycomb Restraint: 1 1/4" 5.3 Orientation (3)

Verify that the Restraint / Support Structure orientation is as shown on the applicable drawings.

Verify visually that components of the hardware match the drawing and that the assembly is per the drawing.

(See Note 3) 5.4.2 Materials (4B) (See Note 7)-

1) For high strength structural bolting and high strength thru anchor bolts, verify that the hardware matches the drawing. Paint shall be removed, if necessary, for this verification. .

ASTM A325 may be substituted with ASTM A449.

11) For U-bar Restraints, verify that the U-bolt size is per the drawing.

iii) For all other components of the Restraint /

Support Structure, record on Attachment 6.2 thoseheat,numbersorpartnumbersthatarej' legible on the component. For components assembled in shop, only the assembly

  • component in the assembly need be recorded.

Paint need not be removed.

5.4.3 Installation (4C) i) Level and Plumb: Verify by measurement that the Restraint / Support Structure is level and plumb within the tolerances indicated below.

Support Structure, Hard Restraint. Honeycomb Restraint: 1 1/8" per foot with maximum deviation not to exceed 1/2" over total piece dimension.

Moment Restraints: i 1/4" per foot with maximum deviation not to exceed I" over total piece dimension. Level and plumbness in this case refer to the base plate and capture plate respectively. (See Attachment 6.7)

Unless specified on the drawing, there are no level and plumbness requirements for other Restraints.

11) Dimensions: Verify by measurement, that,the dimensions required to be maintained in the field are within the tolerances given on'the drawing.

Listed below are some generic tolerances:

Spacing between Bumpers in a multi Bumper installation: 1 1/8" On 'c' dimension shown on Bumper Restraint drawings: 2 1/8" On Support Structures: 11/16" for dimensions d(30' 1 1/8" for dimensions )>30' 111) Restraint Specific: Verify that the '-

following Restraint specific requirements are met:

Bumper Restraints: Verify visually that the mating surfaces for which drawing calls out the requirement " Bear", are in contact at edges.

U-Bar Restraints: Verify visually that the bearing plate covers the entire bent portion of the U-bolts. Also verify that the threaded portion of the U-bolt is completely engaged in the clevis.

Cotter Keys: Where Cotter Keys are required by t;ie drawing, verify visually that the ends were sufficiently deformed after insertion to prevent withdrawal.

5.4.4 Base plates (4D) (See Note 6)

Where drawing requires grout, verify visually that grout exists and that mating surfaces are flush. When mating surfaces are not flush, verify, using feeler gages if the following acceptance criteria is met:

l Inside Containment: Gaps between the. base plate ^and the concrete shall be less than 1/16 inch for_ at least 80% of the base plate area. .It may exceed 1/16 inch for a maximum of 20% of plate but shall not be more than 1/2 inch at any point. Acceptance may be established based on percentages of perimeter, rather than area.

  • i Outside Containment: Acceptance criteria is j same as for inside Containment except that "

the maximum permissible gap is 1 inch instead

-of 1/2 inch.

5.4.5 Shimming (4E)

Verify that shimming is per the drawing requirements and the following generic requirements are met: ,,

U-Bar Restraints: Verify that there is'no visible shim between the Restraint bracket and the Support Structure.

Support Structures:

a. Verify visually that shims cover 100% of the mated parts,
b. For bearing connections, verify by measurement that the total thickness of loose (unwelded) shims does not exceed 1/4 inch and that the remaining gap is filled by a single shim welded all around to one of the interfaced plates with a fillet veld. The requirements of Section 5.8 do not apply to these welds.

Moment Restraints:

a. Verify that the requirements of a and b for Support Structures above are met.
b. Verify visually that shims between capture plate and forging, bear fully against the capture plates.

Bumper Restraints: For shims between Bumper Restraints and Support Structures:

a. Verify that the shim thickness is not less than 1/4 inch,
b. Verify that the. shim is either one piece or meets the requirements of Attachment 6.16.

5.4.6 Attachments (4F)

Verify that the instrument bracket attachments to Restraints and Support Structures meet the following requirements:

There shall be a clear ' distance of 1" +

diameter of standard washer or 4" +

dimensions of plate washer at bolts.

1" clear distance from weld edges.

1" clear distance from any edge of Bumper or-tO U-bar attachments.

5.5 Gaps (5) 5.5.1 Environment (SA)

Record Ambient Temperature and the System Conditions.

(i.e.,

pipe empty, filled with water, operating at

__ F,__ psi). This data will be obtained from TUGC0 Operations / Maintenance.

5.5.2 Cap (SB)

Verify by measurement that the gap between the bare pipe and the Restraint is within the tolerances specified on the drawing. If the System is under testing and has a temperature of above 120*F (based on information obtained in 5.5.1 above), record the temperature and the gaps on a Supplemental Information.

Sheet and enter "N/A" on the checklist for this attribute. The discipline engineer will evaluate the gaps for acceptance and issue deviation reports, if appropriate. Listed below are some generic tolerances:

U-Bar Restraints: Gaps on U-Bar Restraints are shown as dimension "1" and "2" on HFT (Hot Functional Testing) data cards or i dimensions D and 6 on G6H App. 7 to MS-94 drawings. Bumper Restraints: Tolerance on "g" -

Moment Restraints: Unless shown otherwise on i the drawing the following tolerances apply:

a. Where design shows 1/16" on both sides, the. total dimensional t'olerance shall be 1/8" ! 1/16"

(e.g. 0" on one side with 1/8" t 1/16" on the other, 1/16" ! 1/32" on

{ both sides or any combination)

b. Where design shows 0" on one side and 1/16" on the other side the sum of both gaps may not exceed 1/8" or be less than 1/32".
c. Where the design shows clearances greater than 1/4", the dimension shown is the minimum.

5.6 Bolted Joints (6) i 5.6.1 Size and Number (6A)

Verify that the number of bolts and the bolt size is per the drawing. Except for Moment Restraints, the required bolt size is 7/8 inch if not shown on the drawing.

5.6.2 Joint Details (6B)

Record joint details on Attachment 6.6. If the drawing does not call for high strength bolts, it is not necessary to remove paint. In such cases if the bolt head has no visible markings indicate so on Attachment O 6.6.

Verify visually that a washer is installed and that it completely covers the hole.

5.6.4 ' Slope (6D)

Verify by measurement that the slope of bolted parts in contact with the bolt head and nut does not exceed 1:20 with respect to a pit.ne normal to the bolt axis. Where the slope is greater, verify visually that a beveled washer has been installed to provide full bearing to the head or nut.

5.6.5 Torque (6E) (See Note 8)

Verification of joint tightness using a wrench as required herein, will be performed only-when the joint is unpainted.

For bearin'g type connections, verify that th'e' bolt or nut cannot be rotated by hand.

Except for Moment Restraints, all connections

,O are-bearing type unless noted otherwise on V- the drawing.

5.6.5 Torque.(Cont'd)

For friction type connections, verify using a calibrated wrench that the bolts are torqued to the value specified in 70% , torque column of Attachment 6.14. Record the wrench number and date of calibration on the Checklist.

For hand tight connections, verify that the  !

bolt or nut cannot be rotated by hand (in the tightening direction).

For bolted connections where a specific torque is called out on the drawing, verify the torque using a calibrated wrench. The value of torque used for this verification will be 70% of tie minimum torque specified.

Record the wrench number and date of calibration on the Checklist.

5.6.6 Locking Device (6F) (See Note 8)

' For bearing type connections utilizing high

' strength bolts of nominal diameter 1 inch and larger, verify visually that there is a jam l nut or the threads have been spoiled to '

prevent the nuts from loosening.

For b'lts o through slotted holes and other g connections noted as hand tight, verify that the nut is hand tight and that there is a jam nut or the threads have been spoiled to ,'

prevent the nut from loosening. Another acceptable locking method is per Attachment 6.5.

V) 5.6.7 Nut Engagement (6G) _

1 Verify that nut of each bolt has proper engagement such that the end of the bolt is not lower than the top of the nut, unless shown otherwise on drawing. When standard 3 nuts are used as jam nuts (outer nut), they -l shall have a minimum thread engagement of one half the nut depth. .]

j 5.7 Anchor Bolts (7) 5.7.1 Richmond Inserts and Embedded Bolts (7A)

1) Size and Number: Verify that the number of bolts and the bolt size is per the drawing.
11) Spacing for field drilled holes: Verify by measuring to the nearest 1/16 inch, that the bolt spacing is as shown on the drawing.

Unless shown otherwise on the drawing, verify that the minimum edge distance requirements of Attachment 6.4 are met. ,

, ) 5.7 Anchor Bolts (Cont'd) 5.7.1 Richmond Inserts and Embedded Bolts (Cont'd) iii) Washer: Verify that a washer is installed and that it completely covers the hole, iv) Slope: Verify by measurement that the slope of surfaces of bolted parts in contact with the bolt head or nut does not exceed 1:20 with respect to a plane normal to the bolt axis. Where the slope is greater, verify that a beveled washer has been installed per Attachment 6.15.

v) Torque: Verify that the nut or the bolt cannot be rotated by hand.

vi) Nut Engagement: Verify nut of each embedded bolt has proper engagement such that the end of the anchor bolt is not lower than the top of the nut, unless shown otherwise on drawing. '-

,_ vii) Thread Engagement for Richmond Inserts k_) The verification of thread engagement as required in this section will be performed for each Richmond insert (or screw anchor) specifically called out on the drawing.

Where drawing allows either a Richmond or an embedded anchor, the verification of this section will be performed on those anchors which can be identified to be Richmond inserts (i.e., where bolts rather than studs have been used in Richmond inserts). {

Verify the thread engagement for Richmond inserts is at least 2 times the bolt diameter ,

plus 1/8 inch. Thread engagement shall be {

calculated as shown below using bolt length  !

Where bolt / stud length is not specified on the design drawing, the length shall be determined by an ultrasonic examination performed by Brown & Root in accordance with Reference QI-QAP-10.2-4B. s Verify by measurement to the nearest 1/8 inch, anchor bolt diameter.

Larger diameter Hilti-Kwik bolts and Super Kwik bolts may be substituted for the bolt sizes shown on the drawings.

Hilti Super Kwik bolts may be substituted for Hilti Kwik bolts provided they are equal in diameter. This does not apply when 8" minimum embedment is required for a 1" O HKB.

Determine anchor bolt length by the letter designation stamped on the top of the bolt (See Attachment 6.8). Bolts of 19 inch length and greater may be stamped with a ,'

number corresponding to the bolt length in inches instead of the stamped letter designation. Remove paint, if necessary.

Super kwik bolts are identified by a star in addition to the letter designation.

If length identification is not verifiable or visible, an ultrasonic examination shall be performed in accordance with QI-QAP-10.2-4B.

(Brown & Root shall perform UT and provida copy of results to inspector for inclusion in verification package). Once ultrasonic examination is complete, record actual length on inspection checklist in remarks column and ,

report as deviation. i Verify that type, size and number of anchors match those shown on the drawing.

ii) Embedment Length:

Verify that the Hilti anchor bolt embedment plus the nut thickness (from Attachment 6.8) for the anchor being measured is not less then the "Embedment length" or " Minimum Embedment Length" identified on the drawings; or lacking such information the " Minimum Embedment" identified on Attachment 6.8.*

Determine the Hilti anchor bolt embedment by measuring the projected length of the anchor bolt from the structural concrete ** surface.

Subtract the anchor projected length from the designated anchor length to arrive at the Hilti anchor bolt embedment. The designated anchor length is that shown on Attachment 6.8, unless the stamp is indistinguishable in which case the U.T. recorded length should be used.

Requirement of minimum embedment for larger -

diameter Hilti Kwik bolts or Super Kwik bolts when substituted for bolts specified on the drawings, is as shown on the drawings or 4 1/2 diameters for Hilti bolts and 6 1/2 diameters for Hilti Super Kwik bolts whichever,is greater, minus the height of oge nut of the size of the Hilti being measured.'

iii) Bolt Spacing Verify for each Hilti, center-to-center spacing of adjacent anchors of the same size (not on the same fixture) is not less than 10 anchor diameters unless specified on drawing.

For unequal sized bolts, the minimum spacing (unless shown on dwg) is as shown on Attachment 6.9.

Verify spacing of each Hilti to existing Richmond Inserts, concrete edges, abandoned holes, Hilti anchors or embedded anchor bolts that have been cut off meet the minimum spacing requirements specified in Attachment 6.10, unless shown otherwise' en the drawing.

When the letter designation stamped on Hilti bolt is not identified by Attachment 6.8 the following Hilti bolt attributes regardinF length of Hilti's can conservatively be inspected by using Attachment 6.13.

In floor and roof areas where topping has been placed, the thickness of the topping shall be taken into account in determining the projected length. In these areas the thickness of the topping shall be added to the measured projected length taken from the  !

g-- concrete surface. The inspector shall ensure that a measurement

( for topping is provided in the inspection package for all floor or j roof slabs. This value may be "0" if the floor or roof has been i monolithically placed.

Verify that the minimum distance o'f each Hilti anchor to any adjacent embedded plate meets the following requirements of Attachment 6.1.'.

a. Attachments located within 12 inches of a Hilti anchor bolt shall have spacing as shown on Attachment 6.11.
b. For embedded plates without anyj attachment within 12 inch clearance distance, the anchor may be as close-as practical to the' edge of the plate without damage to the plate.

iv) Angularity Verify.that each Hilti installed has its longitudinal axis within 6' from the ,*

perpendicular of the surface it gets into.

Attachment 6.12 gives inspection requirements.

v) Concrete Damage Verify visually.that structural concrete surrounding the fixture is not broken or spalled as a result of installing the Hilti anchor. If spalling exists, verify the depth does not exceed the following:

Verify that nut and washer of each Hilti anchor has proper bearing and that a minimum i of one washer (standard or plate) has been provided. The washer shall completely cover

.the hole in the baseplate. Verify by measurement that the slope of surfaces of bolted parts in contact with the bolt head does not exceed 1:20 with respect to a plane normal to the bolt axis. Where the slope is greater, verify that a beveled washer has been installed per Attachment 6.15.

Verify that no visible weld has been made to the anchor bolt or nut.

Prior to start of inspection assure that welds are clean and free of any foreign material that may preclude an adequate visual inspection. Coatings need not be removed. If, however, the inspector feels that an adequate weld inspection cannot be performed due to erenssive and/or uneven coating application, he/she shall seek the concurrence of the Level III inspector and then have the coating removed. When it is necessary to measure weld dimensions during the course of an inspection, such dimensions should be determined as accurately as is practically achievable using standard weld inspection ,

j tools.

Weld inspections are required on field welds only. A Weld Inspeccion Documentation Card and/or a CB&I Checklist may be included in the Reinspection Package. If it shows any welds in addition to the field welds on the drawing, inspect these additional welds as field welds.

Ten (10) or more rounded indications in any six (6) square inches of surface with the major dimension of this area not to exceed six (6) inches with the area taken in the most unfavorable location relative to the indications being evaluated are unacceptable.

viii) Weld Rust (for Stainless Steel Only): Verify that the weld and the base material two feet on both sides of the weld have no rust colored corrosion product build up which may be linear or localized. Thin films of surface rust are acceptable.

1. 'In the course of inspection the inspector shall note any item not covered by reinspection / verification which appear out'of the ordinary as related to the construction of the inspected item or surrounding area. Note such in the remarks column of inspection checklist and initiate an out-of-scope observation.

This observation shall be listed on package table of contents and stored with the package in records.

2. Numbers in parenthesis refer to the attributes on the inspection checklist.
3. If some components shown on the drawing are missing, initiate a deviation report against " Components".

If all the component shown on the drawing exist, but deviate from other requirements, a deviation report against the attribute " Components" will be written only if the deviation is not covered by another attribute on the checklist.

Example Drawing requires two field welded stiffeners. One stiffener plate is missing.. A deviation report will be. ,

written for " Components". If stiffener plate is missing, any' field welds or dimensions associated with subject stiffener n plates would also be missing. Inspector shall reject other D) affected attributes referencing the deviation report initiated for " Components".

The intent of this note is to avoid multiple deviation reports tha's may be caused by only one deviant condition.

4. ' Drawing, as referenced here, includes all the associated change documents (i.e. DCA's and CMC's). Information shown on change documents supersedes the drawing requirements.
5. The inspector should record inspection tool code from Attachment 6.20 on inspection checklist for those tools used in the inspection. The information is for possible future use and is not a requirement for these inspections.
6. Scope of this QI does not include configuration of grout.
7. ASTM A325, A490, A449, A320 are high strength bolts. A307 is not a high strength bolt.
8. For each bolted joint, the Engineer will indicate the type of joint (i.e., bearing, friction or hand tight) on Attachment 6.6 included in the packaFe.
