DEVELOPMENT abo USE OF DAP Revisicm 4 PROCEDURES Am DISCIPLINE INSTRUCTIONS 1.0 PURPOSE This procedure defines the requirements for procedures to be used to implement the DAP Quality Assurance Program and to execute the Design Adequacy Program. It also defines the requirements for the development, review, approval, implementation, and control of those procedures.

2.0 SCCPE This procedure addresses procedures applicable to the entire DAP scope and detailed instructions for specific Disciplines required for the performance of the DAP work scope. Procedures applicabla to overall CPRT activities l[

are included in the CPRT Procedures Manual.

3.0 DEFINITIONS AND RESPONSIBILITIES 3.1 Definitions 3.1.1 DAP Procedures DAP Procedures provide the basic instructions for per-formance of DAP wor)c. These procedures implement the DAP and those parts of the CPRT procedures that apply to the DAP.

3.1.4 DAP Disciplines DAP Disciplines are specific areas of DAP work which have been defined based on type. Together, the DAP Disciplines comprise all work to be performed by the DAP. The follow- lk ing is a list of the Disciplines and their abbreviations.

.i.1.2.1 Page Numbering The cover sheet and table of contents shell be numbered "i" and "li" respectively. Each page in the body of the procedure or instruction shall be numbered sequentially and include identification of the total number of pages in the k body of the procedure (i.e., _X._ of _Y._). Page numbers for attachments shall consist of the attachment letter plus the s

sheet number (e.g., A-1, A-2, etc.).

4.1.2.2 Standard Page Format O

The body of each procedure shall be prepared using the standard form inc.Luded as Attachment E to this procedure.

Discipline Instructions are prepared on a similar form (Attachment F).

4.1.3 Revisions Procedure revisions shall generally be made by revir.ng the entire procedure with vertical lines at the right hand margin of eac.. page to indicate changes. For very simple or limited change, a'ddenda may be used in lieu of complete revision. Such cases would typically be limited to about two .ines on each of two pages. A maximum of two addenda are permitted between complete revisions. Each affected' page shall be identified in the addendum. Revisions and addenda shall be developed and controlled in accordance g

4.2 DAP Procedurg 4.2.1 Procedure Identification When the need for a procedure or revision is identified, the DAP Manager either prepares the procedure or revision, or assigns its development to someone else. For original procedures, he assigns a procedure number by entering a new number and title in the Procedures Index. He subsequently monitors development of the procedure through its approval.

4.2.2 Procedura Preparation The person assigned prepares the procedure using the format of Attachment A as guidance. In general, the format of Attachment A should be followed, but the DAP Manager may allow variations to allow for specific needs. Interfaces and required interface actions are explicitly identified in the procedure. The procedure should also inclucie ac-captance criteria when appropriate.

4.2.3 Procedure Review

As the procedure or procedure revision nears completion, a reviewer is assigned and a draft copy is distributed.

delegate this responsibility. The reviewer ensures that the procedure is complete, meets program requirements, and is consistent with program. logic. After agreement between the preparer and reviewer, a final revision is prepared.

The preparer and reviewer sign the appropriate portions of the procedure cover sheet (see Attachment D), which is forwarded with the procedure to the DAP Manager for review.

4.2.4 DAP Manager Review The DAP Manager review includes review of all interface requirements. Yhen satisfied that the procedure meets CPRT and DAP requirements in a manner ' consistent with program logic, he recommends approval to the Review Team Leader.

The DAP Manager indicates this recommendation by signing and dating the cover sheet. If the DAP Manager performed both of the reviews required by Sections 4.2.3 and 4.2.4, only one signature is required.

4.2.5 Review Team Leader Approval The Review Team Leader reviews and indicates approval of the procedure by signing and dating.the cover sheet. The approved procedure is returned to the DAP Manager.

4.2.6 Procedure Issuance The DAP Manager files a copy of the procedure in the Master Procedures Manual, updates the table of contents, and files the Distribution Sheet (Attachment C) in the DAP Procedure Distribution Log. He distributes the procedure by O TN-85-6262/10 Page 8 of 15

The preparer and reviewer sign the appropriate portions of the instruction cover sheet (see Attachment D) which is forwarded with the instruction to the Discipline Coordinator for review.

The Discipline Coordinator indicates this recommendation by signing and dating the cover sheat. If the Discipline coordinator performs both of the reviews required by 4.3.3 and 4.3.4, only one signature is required. In the case of Project Management /Multidiscipline dis, the DAP Manager shall perform the review activities of tha Discipline coordinator discussed in paragraphs 4.3.3 and 4.3.4.

4.3.5 Approval The DAP Manager reviews and indicates approval of Disci-pline Instructions (other than Project Management / Multi-discipline dis) by signing and dating the cover sheet. The Review Team Leader reviews and indicates approval of g

4.3.6 Instruction Issuance The DAP Manager files a copy of the instruction in the Dirmipline Instruction Manual, updates the table of contents, and files the Distribution Sheet (Attachment C) in the Discipline Instruction Distribution Log. Discipline Coordinator o Appropriate files O 4.4 Interim Use Procedures The Design Adequacy Manager say, when he deems it appro-priate, direct that a procedure or procedura revision be implemented on an interin use basis. Situations where interim use procedures are appropriate include, but are not limited to, those circumstances where a period of " trial use" is necessary to test the adequacy of a procedure or where changed conditions require the immediate irsuance of a procedure prior to completion of the approval process required by this procedure. A procedure that is issued ior interim use shall be clearly marked as such and the transmittal memorandum shall also clearly state that the procedure is for interim use only. PROCEDURES AND DISCIPLINE INSTRUCTIONS under an interin use procedure shall be. brought into conformance with the permanent replacement procedure within 30 days of its approval. Because work done under an interim use procedure is brought into conformance with the perma-nont procedure, it is not necessary that a copy of the interim use procedure be retained in the DAP files. A controlled Distribution Log shall be maintained for each procedure issued for interim use and each person receiving an interim procedure shall receive a copy of the permanent ,

procedure.

4.5 EFFECTIVE DATES Original issuances- of DAP Procedures and Discipline Instructions become effective on approval unless otherwise k

stated in the procedure or instruction. The DAP Manager shall provide direction regarding the extent to which newly issued procedures are to be applied retroactively. Unless such direction is provided, procedures shall not be required to be applied retroactively. Discipline coordi-nators shall provide direction regarding retroactive application of Discipline Instructions.

The foregoing shall not prevent the retroactive application of a uroced-ure (that is not required to be applied retroactive v) should the user of the procedure deem it appropriate to b so.

Ravised procedures (including addenda to procedures) become effective five (5) working days after approval unless otherwise stated in the procedure. Unless the procedures specifically requires retroactive application of the O TN-85-6262/lO Page 13 of 15  ;

  • o For work completed (approved) prior to the approval date of the revised procedure, no revision to the werk shall be necessary unless otherwise directed by the DAP Manager.

o For work completed (approved) within 30 calendar days of the revised procedure approval date, the affected work may be completed using either the l superceded procedure or the revised procedure. ,

o For work that was previously completed under a superceded A*evision of a procedure and that is being revised (when a revised procedure is in s

effect), the revision of the work shall include revisions necessary to bring the work into conformance with the revised procedure. The DAP Manager may grant exemptions to this requirement.

o For werk completed more than 30 calendar days after the approval a date of a revised procedure, the work shall be brought into conformance with the revised procedure prior to its approval unless an exemption is granted by Manager. the .DAP 5.O DOCUMENTATION 5.1 Distribution Loa Attachment C is a sample page of the Procedures / Instruction Distribution Log. One such sheet shall be filed in the log for each DAP Procedure and Discipline Instruction defined.

Implementation is in accordance with section 4.2.6 for DAP Procedures and Section 4.3.6 for Discipline Instructions.

5.2 Procedure Cover Sheets Attachment D is a cover-sheet form which is used for each DAP Procedure or Discipline Instruction to be developed.

Use of the form is in accordance with Sections 4.2.3 and 4.2.5 for DA~P Procedures and Sections 4.3.3 and 4.3.5 for Discipline Instructions.
