1.0 PURPOSE To provide the methods to be used and the accept / reject criteria for the reinspection of HVAC Duct Supports.

2.0 APPLICABILIIT This instruction applies to installed and final QC accepted HVAC duct supports in Units 1, 2 and common areas. The population is defined'in the Population Description for HVAC Duct Supports.

) 4.1 Responsibilities 4.1.1 QA/QC Discipline Engineers The QA/QC Discipline Engineers prepare the. inspection instructions and checklist delineating reinspection requirements, attributes, and' accept / reject criteria.

4.1.2 QA/QC Inspectors QA/QC Inspectors perform and document reinspection / verifications in accord with these instructions and reference 3.2.

4.2 Policy 4.2.1 Activities performed under this instruction shall conform to the policies contained in the latest Comanche Peak Response Team Program Plan and Issue-Specific Action Plan VII.c.

1. Support Location j

IA. Support Locations Along the Axis of the Duct Verify that the support is installed within 16" of the location shown on the duct support location plan drawing. (See Note #3). Support locations may be described by any dimension (e.g., to an adjacent support, a wall, a column line or a duct). However, only the dimension that locates the support along the.

l axis of the duct shall be verified. This measurement shall be made to the centerline of the primary support members of the frame.

IB. Support Clearance Check Check for any contact between the duct support and other installed items. Where contact exists, the inspector shall provide a sketch showing the relative location of the contact and as much information as is available.to identify the item touching the duct support. Attachment 6.5shallbeusedtoprovidesuch a sketch. This item is for information only, and is  !

not an accept / reject criterion.

2. Support Frame Configuration and Orientation 2A. General Configuration Verify that the general configuration and orientation y of the installed duct support matches that shown on the duct support detail drawing when viewed in the direction (s) specified. Also, all members shown on the duct support detail drawing must be accounted for in the installed support and vice versa.

2B. Member Orientation Verify that orientations of installed members match the duct support detail drawing (e.g., the direction of toes and flats of angle and channel members, skewed member planes, etc.).

2C. Member Size Verify that installed members are the same as shown on the duct support detail drawing. Excerpts from the AISC manual are given in Attachment 6.6 to aid in verifying member sizes where it is not intuitive from the shape description. (Verification of bolting is covered in Sections 4. 5. and 6.).

2D. Member Lengths and Dimensions Verify member lengths and all other dimensions that describe the lengths and positions of members in the support frame (! 1/2"). (Verification of bolting is covered in Sections 4., 5. and 6.).

2E. Minimum Edge Distance (In Steel Members)

Verify that the minimum edge distance for holes in duct support members is in accord with AISC requirements unless otherwise specified on the duct support detail drawing. Attachment 6.7 is a reproduction of AISC Table 1.16.5.1 which delineates these requirements.

Where holes in duct. support members are for S/16" diameter bolts, the inspector shall record all actual edge distances less than 1/2" on the inspection checklist. This item is for information only and does not ,

constitute an accept / reject criterion.

For holes in duct support members fabricated l

of sheet metal, verify that the edge distance '

is equal to or greater than the value listed on the duct support detail drawing.

All edge distances shall be measured from the centerlines of existing bolts.

2F. Welded Attachments to Embedded plates 2F.1 Attachments within " cantilever" regions i Verify that duct support members welded to embedded plates do not attach in " cantilever regions" of plates as shown in Attachment 6.8. If an attachment (s) is made within the cantilever region (s), it is acceptable provided at least one of the following conditions apply:

include as much information as is available to identify items that are not part of the duct support in question. Attachment 6.10 shall be used to provide such sketches.

2G. Support provided by Existing Structure.

Verify that support for the duct (s) is provided by the duct support' structure unless otherwise specified on the duct support detail drawing.

2H. Attachment to Duct / Equipment Verify that the duct / equipment is/is not connected to i the support as required by the duct support detail i drawing. Verification of. welding / bolting between supports and ducts is covered in Section 3. and 6. I respectively. See Attribute 3A.

2J. Attachment of Other Supports Verify that all other duct supports and items attaching to the duct support in question are referenced on the duct support detail drawing.

2K. Duct Size Verify that the duct size (s) shown on the duct support

(_ detail drawing matches the supported duct (s). See Note

  1. 6. This attribute is acceptable if the difference in duct size is due-to the relative location of the support on a duct transition piece. (Other attributes may not be acceptable.)
3. Welding - General Verify, prior to the start of inspection, that welds are clean of foreign material which would impede an adequate visual inspection (See Note #7). Where it is necessary to measure weld dimensions during the course of an inspection, such dimensions shall be determined as accurately as is practically achievable using standard weld inspection tools.

3A. Weld Location Verify that veld configurations, orientations and locations are as specified on the duct support detail drawings. Account for all welds shown on the duct support detail drawing: missing welds shall be cause I for rejection; additional welds shall be noted, for information only, on the inspection checklist except where additional velds represent a violation of ,

attribute 2H.

Welds larger than 1/8": Fillet welds may be undersized to a maximum of 1/16" provided that the total length of undersized weld is less than 1/4 of the weld length.

Groove weld sizes are verified by assuring that the joint is welded flush except as noted in Attachment 6.11 (page 1 of 3).

Fillet welds may be oversized provided they '

do not int'erfere with mating parts.

3C. Weld Lengths b Verify the length of weld.

Welds less than 3" long may be underlength by 1/8". Welds 3" and longer may be underlength by 1/4".

Welds may be longer than specified on duct support detail drawing provided that the overlength portions of welds do not interfere with mating parts.

Where weld lengths are not specified on duct support detail drawings (i.e., the weld length is equal to the length of the joint),

verify that the overlap of installed members provides the same length of joint intended by the joint configuration shown on the duct support detail drawing.

3D. Fusion Verify that the lengths of visible incomplete fusion do not exceed the following: c O In fillet welds less than 4" long, 1/4" (maximum) incomplete fusion is permissible.

In fillet welds greater than or equal to 4" in length, 3/8" (maximum) incomplete. fusion in any 4" long segment is permissible.

Incomplete fusion in groove welds is not permitted.

3E. Craters Verify that the veld size is met on all welds with underfilled craters. Craters outside.the specified weld length are irrelevant provided there are no cracks visible. through the coated surface (if the weld has been coated) or on the uncoated surface if a coating has not been applied.

3F. Weld Profiles Verify visually that weld profiles are acceptable per Attachment 6.11.

3G. Undercut

[ Verify that undercut, if present, does not exceed the

values set forth in Attachment 6.12. (Also, See Note

  1. 9).

3H. Porosity Considering that only surface porosity whose major surface dimension exceeds 1/16" shall be considered relevant verify the following:

The sum of the diameters of porosity do not exceed 3/8" in any linear inch of weld and shall not exceed 3/4" in any 12" of weld, and If 4 or more pores are aligned the edge to edge separation of the pores shall exceed 1/16".

3J. Overlap If overlap is present, verify that the required weld size and the fusion acceptance criteria have been met.

If fusion in the overlap length cannot be verified, then verify that the length of overlap, in which fusion cannot be verified, does not exceed 3/8" in any 4" l

segment or 1/4" in welds less than 4" long.

m 3. Welding (Cont'd) 3K. Surface Slag Verify that the major surface dimensions of the slag does not exceed 1/8". For isolated slag (that which does not occur more frequently than once in a 3" weld segment), verify that the major surface dimension does not exceed 1/4".

3L. Cracks Verify that there are no cracks visible through the coated surface if the weld has been coated or on the uncoated surface if the coating has not been applied. ,

3M. Welders Identification Symbol When the weld is marked with the welders ID symbol, record the symbol on the attribute inspection checklist. This item is for information only, and is j

4 Concrete Expansion Anchors (Hilti) rs

? 4A. Size and Number V)-

4A.1 Bolt Diameter Verify anchor bolt diameter. This can be accomplished by using a gauge or rule to distinguish nominal diameter sizes ranging between 1/4 and 1-1/4 inch diameter in increments of 1/8 inch.

4A.2 Bolt Type, Length and Quantity Verify that type, length, and number of anchors insts11ed match those shown on the.

duct support' detail drawing. If the length identification is not verifiable or visible, it shall be determined using ultrasonic examination.(See Notes #10 and #11).

4B. Embedment (See Note #12)

Verify that the embedment length is not less than the minimum anchor embedment as indicated on the design drawing (or as shown in Attachment 6.14 when not shown on the design drawing) minus the thickness of one nut of the bolt diameter being addressed. (Nut thicknesses are also given in Attachment 6.14)*.

  • Note: When the letter designation stamped on the Hilti bolt is not identified by Attachment 6.13, the Hilti bolt length can conservatively be inspected by using Attachment 6.14.

4C. Bolt Spacing 4C.1 Hilti to Hilti Verify that for each Hilti, the center-to-center spacing of adjacent anchors meets or exceeds that in Attactment 6.15, unless otherwise specified on the duct support detail drawing.

4C.2 .Hilti to Richmond Bolts, Concrete Edges _and Abandoned. Bolts / Holes. ,

Verify that spacing of each Hilti to existing-f~s Richmond Inserts, concrete edges, abandoned holes, Hilti anchors and embedded anchor bolts that have been cut off meets or exceeds the minimum spacing requirements specified in Attachment 6.16, as applicable, unless shown on the duct support detail drawing.

4C.3 Hilti to Embedded Plate Edges Verify that the minimum distance of each  !

Hilti anchor to any adjacent embedded plate l (or Nelson stud, if available) meets the requirements of Attachment 6.17, unless the plate is unoccupied. See Note #13.  !

4D. Angularity i

Verify that each Hilti anchor installed has its longitudinal axis within 6* from the perpendicular to the surface in which it is installed. Attachment 6.19 gives inspection requirements. i 4E. 4E.2 Sloped Surfaces Verify surfaces of bolted parts in contact with the nut and washer of each Hilti anchor have a slope of no more than 1:20 and that a minimum of one washer (standard or plate) has been provided. The washer shall completely cover the hole in the base plate. A bevel gauge shall be used for determining if the gap is less than 1:20. (See Attachment 6.20).

4E.3 Welding Hilti Bolts Verify that no Anchor Bolts or Nuts have been welded. -

bottomed. To do this, measure the distance from the end of the bolt to the edge (of the  ;

nut) closest to the surface in which the anchor is installed; this dimension shall be less than the applicable length of thread shown in Attachment 6.21. A sketch of the dimension in question is also shown in Attachment 6.21.

5. Richmond Screw Anchors 5A. Minimum Thread Engagement Verify that the length of bolt extending into the insert is greater than or equal to 2 x bolt diameter +

1/8". (i.e. 2 1/8" for 1" O bolts and 3 1/8" for 1 1/2" 9 bolts). This can be determined by subtracting the thickness (es) of the items attached to the concrete by the bolt plus the thickness of grout, if present, from the bolt length shown on the duct support detail drawing. Where bolt lengths are not shown on duct support detail drawings, they must be determined by ultrasonic examination. (See Note #10). Since bolt l lengths obtained by ultrasonic examination include the

extending into the insert.

5. Richmond Screw Anchors (Cont'd)

SB.- Bolt Tightness Verify that bolts threaded into Richmond inserts are hand tight. The bolt shall not rotate.

SC. Insert Spacing Insert clusters - Verify that, when used, 11" 6 inserts used to connect duct support members to the structure, that are part of insert clusters. are not less than 20" (center to center) from inserts in the same cluster used for other plant equipment. Note:

An insert cluster is defined as 2 or more inserts spaced at 18" (maximum).

SD. Sloped Surfaces Verify that surfaces of bolted parts in contact with' the bolt head of the Richmond Insert bolt have a slope of not more than 1:2.0 with respect to a plane normal tp the bolt axis. Where slopes are in excess of 1:20, verify that beveled washers, as required, have been installed. (See Attachment 6.20).

SE.- Material Identification Check for material identification on Richmond bolts.

Record the material identification, when present, on verification package checklist, Attachment 6.1. This item is for information only, and is not an accept / reject criterion.

6. Miscellaneous Polting (other than Hilti Expansion Anchors and Richmond Insert Bolts) 6A. Bolt Size Verify that the diameter of bolts is as specified on the duct support detail drawing. This can be done by using the size of the bolt head in accordance with AISC ,

standards or by measuring the major diameter of the threads (or bolt body) directly.

6B. Bolt Material Type  !

Verify that high strength bolting has been installed in bolted connections for which a torque value is S l specified on the duct support detail drawing. (See Note #14).

6. Miscellaneous Bolting (Cont'd)

The use of high strength bolting in connections for which no torque value is specified on the duct support detail drawing is acceptable, but shall be noted on the inspection checklist.

The use of common bolting, other than in connecting duct / equipment to supports, shall be noted by the inspe: tor on the inspection checklist in all cases. (See Note #16).

6C. Thread Engagement Verify that the ends of bolts are not lower than flush with the tops of nuts.

6D. Washers Verify that all threaded fasteners are provided with washers as specified. on the duct support detail ,

drawing.

Connections for which a torque value is O specified on the duct support detail drawing shall have, at least, one washer installed.

It is acceptable to have washers (or additional washers) installed in bolted connections when not specified on the duct support detail drawing.

6E. Torque / Tightness Verify that bolts for which a torque value is specified l

on the duct support detail drawing are torqued to 70%

of the torque values listed on the duct support detail drawing. (See Note #15). Brited connections for which no torque value is specified on the duct support detail drawing shall be verified as being hand tight.

6F. Sloped Surface Verify that surfaces of bolted parts in contact with the bolt or nut have a slope of not more than 1:20 with respect to a plane normal to the bolt axis. Where slopes are in excess of 1:20, verify that beveled washers, as required, have been installed. (See Attachment 6.20). "

codes should be recorded on Attachment 6.2. Where tools have serial numbers or calibration due dates, these shall be recorded on Attachment 6.2. The information is for possible future use and is not a requirement for these inspections. (Reference Section  !

5.0).

3. All duct. supports are located on duct support location drawings.

Additionally, some duct support detail drawings include a location plan. Support locations shown on detail drawings shall take precedence over the duct support layout drawings. Locations of duct supports shall be verified using dimensions shown on the layout drawings only if no location plan is given on the duct support detail drawing. Any discrepancies between the locations shown on the duct' support detail drawings and the duct layout plan drawings should be noted on the " Supplemental Remarks" Attachment when identified. (Reference Section 1A.).

various lengths which are anchored to concrete surfaces by eubedded Nelson studs. "Large Plates" are those 3/4" x 4'-0" x 8'- 0" or

()' 12'-0", which are anchored to concrete surfaces by embedded Nelson Studs. This includes plates of either of these nominal sizes which were modified to include cut-outs or irregular outside shapes to conform to building configurations. (Reference Section 2F.1).

5. Where the duct support attachmeat centerline is not parallel to the longitudinal centerline of the " strip" place, this dimension shall be the shortest distance between the plate centerline and the intersecting point of the orthogonal centerlines of the duct support attachment. (Reference Section 2F.2).
6. Duct sizes shown on duct support detail drawings are nominal sizes only. (Reference Section 2K., " Welding-General").
7. Coatings need not be removed unless an inspector feels that, due to excessive and/or uneven coating application, he is unable to provide an adequate inspection of. the weld. He shall then contact the Level III inspector, and with his concurrence only, may the coating be removed. (Reference Section 3.).
8. Where weld sizes are not specified on duct support detail drawings, the size of the weld is as follows:"

For welds that connect the ducts / equipment to support:

1. Rectangular duct, vall penetration supports (prefixed C "WP"): 3/32" fillet welds, 1" long (+1",-0"), on 4" centers (21") on two sides of the duct (minimum).

l l- 3/32" fillet welds, 1" long (+1",-0"), on 4" centers (21"), all around the duct.

3. All other rectangular duct, duct supports: 3/32" fillet welds, 1" long (+1",-0"), on 6" centers ( 1") on two sides of the duct (minimum).
4. All other round duct, duct supports: 3/32" fillet l-welds, 1" long (+1",-0"), in four places (tangent points).

For all other welds, the size of the weld shall be taken to be the equal to the thickness of the thinner part joined.

(Reference Section 3B.).  !

9. The total weld equals the cumulative length of both sides.

(Reference Section 3G. and Attachment 6.12).

10. Ultrasonic examinations shall be performed by Brown & Root in accord with Reference 3.3. Once the ultrasonic examination is complete, the. inspector shall record the actual length on the ,

inspection checklist and include the test results in the verification pcckage. (Reference Section 4A.2).

11. Determine the anchor length by the letter designation stamped on the top of the bolt. Remove paint if necessary (See Attachment 6.13). Super Kwik bolts are identified ;y a star in addition to the letter designation. (Reference Section 4A.2).
12. Determine the embedment length of the installed anchor by measuring the projected length of the anchor bolt from the structural concrete surface *. Subtract the projected anchor length from the designated anchor length to obtain the Hilti anchor bolt embedment length. The designated anchor length is that shown in Attachment 6.13, unless an ultrasonic examination was used, in which case the examination length shall be used. (Reference Section 4B.).
  • Note: In floor and roof areas where toppir.g has been placed, the thickness of the topping shall be taken into account in determining the projected length unless shown on the duct support detail drawing. In these areas the thickness of the topping shall be added to the measured projected length taken from the concrete surface. The inspector shall ensure that the thickness of topping is provided by the engineer and recorded on the inspection checklist.
13. An embedded plate is considered unoccupied by other attachments ,

when there are no attachments within 12" on either side of line '

('^ drawn from the Hilti expansion anchor location perpendicular to the plate edge in question. See Attachment 6.18. (Reference Section 4C.3).
