During the period July 7-11, 1986, and on August 14, 1986, members of the NRC

Region I staff conducted a special safety inspection related to the Performance

Appraisal Team (PAT) Report 50-289/86-03 at your facility. The results of

these inspections are documented in the enclosed report. At the conclusion of

the inspection, Mr. R. Conte of my staff summarized the inspection findings for

you and other members of your staff.

In this inspection NRC Region I reviewed the PAT findings to (1) determine which

findings are violations of regulatory requirements, and (2) itemize other

findings requiring more NRC followup. As a result, this inspection necessarily-

focused on the negative aspects of the PAT inspection; the PAT's positive

observations, which were substantial, were not revisited in this inspection.

This Region I review identified several violations of NRC requirements. You

are required to respond to this letter; and,,in so doing, you should follow the

instructions in Appendix A of this letter.

The PAT inspection and this followup inspection detail examples of apparent

failures to properly implement modification control procedures, including

failure to properly evaluate modifications. Since most of the design, techni-

cal review, and safety review work has already been completed for modifications

to be installed in your upcoming Cycle 6 refueling outage, we are concerned

that problems similar to those noted herein may also exist for the Cycle 6

modifications as well as previous safety grade modifications completed before

the October 1985 restart. Accordingly, in your response, please inform us of

any measures taken or planned to assure that other modifications, such as those

c to upgrade the emergency feedwater (EFW) system to safety grade for Cycle 6,

are in fact fully adequate. If any action you plan to take is not complete at

the time of your response, please provide a schedule for completion of such

action.
