APPENDIX A Colt Industries Docket No. 99900300/86-01

NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on June 9-12,1986, it appears that certain of your activities were not performed in accordance with NRC requirements and those -found in your Quality Assurance Manual.

A. Criterion XII of Appendix B to 10 CFR Part 50 states, ," Measures shall be established to assure that tools, gages, instruments and other measuring and testing devices used in activitjes affecting quality are properly controlled, calibrated and adjusted at specified periods to maintain accuracy within necessary limits."

Nonconformances with this requirement are as follows:

1. Section 12.4.6'of you Quality Assurance Manual reads, in part,

" Quality Control Inspection personnel and manufacturing Supervision will, as a part of their daily routine, remove from use all devices found that are overdue for calibration...."

Contrary to the above, no evidence could be found to demonstrate that a weld machine, Asset No. 32171 (Lincoln S/N AC560182), that was due for recalibration during April 1986, had been recalibrated prior to its use on May 29 and 30,1986. (86-01-01)

2. Section 12.4.2.1 of your Quality Assurance Manual reads, in part, "All reference standards, or ' masters' used for calibration must be periodically calibrated, using a standard whose accuracy is trace-able to the National Bureau of Standards or to a known physical constant, when such standards do not exist."

Contrary to the above, a 0 to 1600 ohm Leeds & Northrop bridge (S/N 743053) that is shown in your Reference and Standards Listing of 4/25/86 (Rev. 0) was identified as being calibrated by Rockford Instrument Laboratory with no evidence that a standard traceable to the National Bureau of Standards was used. (86-01-02)

B. Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accom-plished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Section 18.4.5.1 of your Quality Assurance Manual states: " Management

of the- audited groups shall prepare a written reply indicating either the correct'ive action taken or to be taken to resolve the nonconformances disclosed by the audit, or the complete rationale for not taking action.

The replies shall be issued within 30 calendar days of the r'eport issue date."

Contrary to the above, written replies to vendor audits are not consis-

- tently being issued within the required 30-day period. (86-01-03)
