Rockwell International Finw Control Division Docket No. 99901069/86-01 NOTICE OF NONCONFORMANCE 6 .

Based on the results of an NRC inspection conducted on October 20-24,1986 i t appears that certain of your activities were not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Nonconformances with these requirenents are as follows:

1. Subsection 1.1 " Statement of Policy and Authority" of the Quality Assurance Manual (OAM) Revision 19 commits RI to conform to Section III of the ASME Code, ANSI N45.2, and 10 CFR Part 50, Appendix B.

Both Subsection NCA-4134.6 cf Section III of the ASME Code and Section 7 of ANSI N45.2-1977 require that documents, including changes, are distributed to and used at the location where the prescribed activity is performed. .

Subsection 5.1.7 of the QAM states, in part, "The Quality Assurance department is responsible for controlling the distribution of...

RMP's after release. The Quality Assurance departme'nt shall maintain a controlled working file on...RMP's."

Subsection 6.2.3 of the CAM requires that the CA Department distribute Rockwell Method / Process Specifications (RMP) using a Department Receipt Form (DRF) with the DRF being returned to the OA Department to assure that the required action has been completed.

Contrary to the above, it was noted that: (86-01-02)

a. Revision A dated January 31, 1984 to RMP No. 6117C " Primer Coating of Carbon Steel Valves for Nuclear Service (Mobil zinc)" was not at the coating station.

b. Revision B dated April 8,1986 to PMP No. 61360 " Combination GTAW Root Fuse and SMAW Filler Welding of P-1 Materials" was not in manuals No.11 (Nuclear Assembly and Test Supervisor) and No. 17 (Plant Metallurgist).
c. RMP No. 61369, Revision C " Procedure for Welding of Permanent Attachments on Stainless Steel Univalves and Hermavalves for Nuclear Service" was in manual No.11, but a DRF dated November 12, 1985 indicated that RMP No. 61169 was obsolete,
d. RMP No. 61150, Revision A " Assembling and Testing Rockwell-Edward Hermavalves with Leakoff (for Westinghouse)" was missing from manual No. 4 assigned to the OA Department.
2. Subsection 10.5.1 of the OAM states, in part, "The training, qualification and certification of NDE personnel will be accomplished in accordance with

...SNT-TC-1A...."

Section 9.6.1 of SNT-TC-1A states, in part, "The qualification records of the certified individuals shall be maintained and... include the following:

...(4) statement indicating satisfactory completion of training in accord-ance with the employer's written procedure...."

f Contrary to the above, a review of qualification records for nondestructive examination personnel revealed that the records for the Level III examiner did not contain a statement indicating satisfactory completion of training in accordance with OA Instruction No. 40-03-12. (86-01-03)

3. Subsection 12.2.1 of the OAM states, in part " Measuring and testing equip-ment which is found to be out of calibration shall be imediately disap-proved for use and identified as rejected by the Ouality Assurance Department until corrective action has been implemented...."

Sections 15.2 and 15.3 of QA Procedure 761-40-04-12 require stationary measuring equipment that is past due for calibration to be identified with a REJECT tag.

Contrary to the above, a stationary Ph measuring instrument (RI 4 Asset No. 61-3149) was found to be out of calibration and not

identified with a REJECT tag or label. (86-01-04) 4 i 4. RMP No. 61150 " Assembling and Testing Rockwell-Edward Hermavalves i with Leakoff (for Westinghouse)" requires nuclear stainless steel l valves to be hydrostatically tested using Grade A water.

Contrary to the above, nuclear stainless valves (Assembly Lot H7510) 1 were hydrostatically tested on October 16, 1986, using Grade B rather l i

5. Subsection 7.1.4 of the OAM states, in part, "The Purchasing Manager shall place Purchase Orders with only those vendors on the Approved Vendor List."

Contrary to the above, P0 M4454 was issued to Page Wilson for hardness test samples on August 6,1986,,but Page Wilson was not on RI's Approved Vendors List. (86-01-06)

6. Subsection 9.3.1.5 of the OAM states, in part, "The hardness tests shall be performed and the results recorded on an Inspection Card by Quality Control." T Contrary to the above, hardness test results on stem and disc matIr.ial for nuclear valves on assembly lot numbers H , 4175, 6866, 7011, 7191, 5824, and 5726 were not recorded on an Inspection Card. (86-01-07)
7. Section 6 of ANSI N45.2-1977 states, in part, " Activities affecting ouality shall be prescribed by documented instructions, procedures...

of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures...."

Contrary to the above, documented instructions / procedures did not exist for the accomplishment of specific inspection requirements (i.e., hardness checks) on the "In-process Heat Treati.,ent" report.
