APPENDIX A BBC Brown Boveri, Incorporated Docket No. 99900835/86-01 NOTICE OF NONCONFORMANCE I
Based on the results of an NRC inspection conducted on December 2-5, 1986, it appears that certain of your activities were not conducted in accordance with NRC requirements.
A.
Criterion VII of Appendix B to 10 CFR 50 states in part:
" Measures shall be established to assure that purchased material, equip-ment, and services, whether purchased directly or through contractors and subcontractors, conform to the procurement documents. These measures shall include provisions, as appropriate, for source evaluation

i and. selection, objective evidence of quality furnished by the contractor or subcontractor, inspection at the centractor or subcontractor source, and examiration of products upon delivery. The effectiveness of the control of quality by contractors and subcontractors shall be assessed by the applicant or designee at intervals consistent with the importance, l
complexity, and quantity of the product cr services."
Contrary to the above, BBC has not established controls necessary to ensure charging motors purchased from subvendors conform to BBC drawings.
Additionally, Von Weise Gear Company, Black and Decker, and t,amb Motors, the manufacturers of the charging motors, are not on the BBC approved vendors list.

Criterion III of Appendix B to 10 CFR 50 states,
" Measures shall also be established for the selection and review for suitability of application of materials, parts equipment, and processes that are essential to the safety-related functions of the structures, systems and components."
"Where a test program is used to verify the adequacy of a specific l
design feature in lieu of other verifying or checking prccesses, it shall include suitable qualifications testing of a prototype unit under the most adverse design conditions."
" Design changes, including field changes, shall be subject to design control measures commensurate with those applied to the original desinn and be approved by the organization that performed the criginal desico unless the applicant designates another responsible organization."
Contrary to the above, BBC has not established controls recessary to ensure environmental qualification when similar parts are substituted for those originally installed in qualified circuit breakers. Additionally, BBC has not established controls necessary to ensure design changes made by charging motor subvendors are reviewed for their effect on equipment quali-fication.
