I APPENDIX B Telemecanique, Inc.

99901011/85-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on December 17 and 31,1985, it appears that certain of your activities were not conducted in acc]rdance with NRC requirements.

A.

Criterion V of Appendix B to 10 CFR Part 50 states:

"Activitus affecting quality shall be prescribed by documented instructions, procedur es, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Criterion VIII of Appendix B to 10 CFR Part 50 states:

" Measures shall be established for the identification and control of materials, parts, and components, including partially fabricated assemblies.

These measures shall assure that. identification of the item is maintained by heat number, part number, serial number, or other appropriate means, either on the item or on records traceable to the item, as required throughout fabrication, erection, installation, and use of the item.

These identification and control measures shall be designed to prevent the use of incorrect or defective material, parts, and components."

Paragraph 2, QAM 8, Rev. 6, of Gould's Quality Assurance Manual states, in part, " Controls shall be established to assure that only correct and accepted items are used in the manufacture of plant products."

Contrary to the above, terminal screws used in the wiring of size one and two combination reversing motor starters were not specified on the naterial lists used for fabrication of those units. As a result, incorrect (too long) screws were used which cracked the plastic barrier plate and subsequently jammed the starter.

Criterion VII of Appendix B to 10 CFR Part 50 states, in part:

" Measures shall be established to assure that purchased material, equipment, and services, whether purchased directly or through contractors and subcontractors conform to the procurement documents."

Paragraph 2, OAM 7, Rev. 6 of the Telemeca nique (formerly Gould) Quality Assurance Manual states, in part, "The procurement of items or services shall be controlled to assure conformanca by vendors to the specified requirements of Purchase Orders."

Contrary to the above, Telemecanique (formerly Gould) did not assure that circuit breakers purchased from ITE Electrical Products conformed to the procurement documents.

Specifically, Gould did not ensure ITE effectively implemented a Quality Assurance program to comply with the requirements of 10 CFR 50 Appendix B and 10 CFR Part 21 as applicable to their manufacture of 480 volt molded case circuit breakers.
