APPENDIX A General Electric Company Docket No. 99900403/86-03 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on August 11 and 12,1986 it appears that certain of your activities were not conducted in accordance with NRC requirements.
- 1. Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
The General Electric (GE) Nuclear Services Procedure (NSP) 30-03 in paragraph 3.3.2 states in part that discrepancies between GE and the customer shall be resolved prior to formal acceptance of the purchase order. Resolution of the discrepancies will be reached by GE management, quality assurance and customer and shall be documented by purchase order change or other written documentation signed by the customer.
Contrary to the above, GE filled Vermont Yankee (VY) PO# 2260 Supp. I dated August 9,1984, for 200 safety-related scram solenoid pilot valve refurbish-ment kits (kits) type FV 204-139 with kits which were not safety-related without reconciling the variance between the safety-related kits ordered by VY and the kits which were supplied. GE neither issued a purchase order change order documenting the variance nor obtained acknowledgement from VY that they received kits which were not safety-related rather than the safety-related kits which were ordered (86-03-01).
