'e APPENDIX A United States Testing Company Unitech Services Group Docket No. 99901085/87-01 NOTICE OF NONCONFORMANCE e

Based on the results of an NRC inspection conducted on March 9-11, 1987, it appears that certain of your activities were not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished

in accordance with these instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for detemining that important activities have been J satisfactory accomplished."

Nonconformances with these requirements are as follows:

1. Section 6.5.3 of Unitech Services Group (USG) Procedure No. 2.3.1,

" Qualification and Certification of Non-Destructive Testing Personnel,"

dated March 1986, states, in part, " Examinations for Levels I, II and III shall be administered by a Level III NDE Examiner or his designee."

Section 8.4.1 of SNT-TC-1A (June 1980 Edition) states, in part, "An NDT Level III shall be responsible for the administration and grading of examinations for NDT Level I and II personnel."

Contrary to the above, it was noted that examinations were not administered by a Level III examiner or a designated representative for the fnllowing NDE examiners: Nos. 818 (July 1985), G. Tope (February 1987), 767 (August 1985), R. Jones (September 1986),38

< (March 1986), and 40 (March 1986). (87-01-01)

2. Section 6.7.1 of USG Procedure No. 2.3.1 states, in part, "A record of personnel certifications shall be maintained...in the corporate office."

Section 6.7.2 of USG Procedure No. 2.3.1 states, in part, "... records shall contain the following:...e) results of visual and color vision examination...."

Section 9.6.1 of SNT-TC-1A (June 1980 Edition) states, in part, "The qualification records...shall be maintained...and should include the following:...(5) results of the physical examination...."
