This refers to the inspection conducted by E. H. Trottier and W. P. Haass of this office on June 9-12, 1986, of your facility in Beloit, Wisconsin, and to the discussions of our findings with you and members of your staff at the conclusion of the inspection.
This inspection was made as a result of the failure of the air scavenging blower on a standby diesel engine at Peach Bottom Atomic Power Station in January 1986, and main bearing failures in standby diesel engines at Fermi l
2 Nuclear Station in late 1985 and early 1986. The inspection was also con-l cerned with the adequacy of your quality assurance program and your adherence i
to the requirements of 10 CFR Part 21. Areas examined during the inspection and our findings are discussed in the enclosed report. Within these areas,-
the inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspector.
During the inspection it was found that a welding machine remained in use subsequent to its calibration expiration date; a Leeds & Northrop bridge was calibrated against a standard not traceable to an NBS standard; the lack of a vendor response within 30 days of an audit was not identified until the FMED quarterly audit summary report was issued; and vendor inspection reports were not properly reviewed by FMED's Material Review Board. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.
Please provide us within 30 days from the date of this letter a written statement' containing:
(1) a description of steps that have been or will be taken to correct these items; (2) a description of steps that have been or will be taken to prevent recurrence; and (3) the dates your corrective actions and preventive measures were or will be completed. Consideration may be given to extending your response time for good cause shown.
