s APPENDIX B Enerfab, Incorporated

. Docket No. 99901064/86-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on September 15-18, 1986, it appears that certain of your activities were not conducted in accordance with NRC requirements.

A. Criterion V of. Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Contrary to the above, the following two instances of your failure to establish procedures relating to activities affecting quality were noted (86-01-02):

1. The lack of procedures to insure that handling, shipping and storage activities were accomplished in accordance with specified procurement documents and/or standards (i.e., ANSI N45.2.2-1978) and,
2. The lack of procedures to insure that appropriate quantitative or qualitative acceptance criteria were met for items requiring receiving inspection.

B. Criterion VII of Appendix B to 10 CFR Part 50 states, in part: ...The effectiveness of the control of quality by contractors shall be assessed by the applicant at intervals consistent with the importance, complexity, and quantity of the product or services...."

Section 8.7.3 of the Enerfab Quality Assurance Manual (QAM) states, in

" ... Items and services shall be accepted only from vendors who part:

appear on the Approved Vendors List...."

Contrary to the above, Enerfab failed to ensure that Cuality Techniques, Incorporated (QTI) was on the Approved Vendors List. Services were performed by QTI for the calibration of gage blocks (standards) which were then used by Enerfab for the calibration of their own measuring and test equipment. (86-01-03)
